About this file
This document is a Small Business Subcontracting Plan attachment to be completed by an offeror for a procurement opportunity through the Virginia Department of Small Business and Supplier Diversity (DSBSD). The form requires potential contractors to disclose their intentions regarding subcontracting, specifically focusing on the utilization of DSBSD-certified small businesses, including women-owned, minority-owned, and service-disabled veteran-owned businesses. The document provides space for listing up to five potential subcontractors, with fields for company name, contact information, SBSD certification details, contract value, and description of work.
The form emphasizes the importance of small business participation, noting that proposed subcontractor participation will be incorporated into the subsequent contract and become a contractual requirement. Failure to achieve the proposed participation levels may result in a breach of contract. The document defines a small business as one certified by DSBSD, with certification applications available online. Offerors must indicate whether they plan to use subcontractors or complete all work themselves, and those planning to use subcontractors must provide detailed information about each potential subcontractor to demonstrate their commitment to supporting small businesses in the procurement process.
