SCOPE OF WORK
OPHTHALMIC INSTRUMENTS
2024-with 4 option years
DESCRIPTION/SPECIFICATION/WORK STATEMENT
SCOPE: This statement of work request maintenance services on the specialty optometry ophthalmic medical equipment at Optometry Dept. within Northern Navajo Medical Center (NNMC). Two schedule preventative maintenance will be within the Period of Performance and shall begin: Base Year with 4 option years to extend
HOURS OF WORK: Contractor/Subcontractor shall perform work under during normal business working hours, 8:00am - 4:30pm, Monday through Friday, weekend coverage is not required.
SCHEDULING OF PREVENTATIVE MAINTENANCE: The preventative maintenance inspection shall be performed during the specific month or designated by the Contracting Officer’s Representative (COR). COR and Optometry Dept Supervisor must be notified in advance for mutual scheduling of each preventative maintenance inspection. The equipment will be made available to the contractor at the pre-arranged time and period.
PREVENTATIVE MAINTENANCE SERVICE: Contractor/Subcontractor shall provide all materials, tools, test equipment, parts and manpower required for completion of the scheduled preventive maintenance and/or repair service. At completion of each preventative maintenance or repair. Preventative maintenance service shall include cleaning instruments interior/exterior, lubrication, and calibration, alignment, cleaning of optical focal points, minor repairs and adjustments of moving parts. Some small defective parts need replacement to ensure proper operation. Return the instrument to the condition and performance level specified by the equipment manufacturer. The preventative maintenance procedure must be at least equivalent in scope to that provided in the manufacturer’s service manual.

CALIBRATION: Contractor/Subcontractor shall perform calibration using verifiable calibrated test equipment on updated control solution/regents to adjust the device to operate within manufacturers specifications to assure proper operation. Adjust equipment’s and its modules to a performance level as for the device when new.
REPAIR/EMERGENCY SERVICE: All cost related to equipment repair service will be covered under this contract. Consultation service will be available via telephone on 40-business hours, 5-day per week basis. Only requests for repair service initiated by the COR will be accepted.
PARTS: Some small parts are included on minor pm/repairs and adjustments for the preventative maintenance and unlimited call back repair service are to be included in the contract cost. All parts shipment cost required for repair of the equipment, including parts returned and restocking charges are included in the contract cost. The contractor is responsible for installation of all parts.
PARTS EXCLUSION: Supplies, consumable (and complete accessory assemblies) are excluded from contract coverage. Parts determined by mutual agreement of the contractor’s representative and the COR to have been damaged by accident, abuse or acts of nature are also excluded from the contract cost. All required parts and related cost not covered by the conditions of the contract shall be furnished and installed only after receiving authorization from the Contracting Officer, and covered by issuance of a separate purchase order, prior to installation of said parts.
MODIFICATIONS AND UPDATES: Contractor shall notify the Contracting Officer’s Representative (COR) of all manufacturers recommended modifications and updates to the equipment covered by the contract. All modifications & updates must be authorized by the COR; determination will be made for cost coverage as part of the contract or for payment by separate purchase order. All modifications authorized must be completed within 60 days of the contractor’s receipt of the authorization.
CHECKING-IN: Contractor, upon entering the facility and before performing any work must physically check-in with the COR, and will be informed of any additional check-in requirements of the facility. Upon completion of the work and prior to leaving the facility, the contractor must again see the COR, to get their signature on the service report and to leave a legible, photo copy of the service report with him or her.
SERVICE REPORTS: The service report becomes part of the permanent equipment history file; it shall indicate the preventive maintenance and calibration procedure used, manufacturer’s modifications and updates completed, description of all work performed including number of man hours, all parts replaced with part number or nomenclature. The service report should also indicate recommendations for future modifications, updates, parts replacement, operator instruction or procedural changes.
CONTRACTS: Contractor shall provide to the COR Contracting Officer’s Representative, the name of the service personnel who will be working on the equipment as well as the name of the person responsible for contract administration. Telephone numbers for service calls and contract administration shall also be provided to the COR Contracting Officer’s Representative. If applicable, the contractor shall provide the correct customer/account/reference numbers for each equipment location.
CONTRACT MONITORING: The COR Contracting Officer’s Representative will monitor the contract by review of the information provided with the bids, evaluation of actual work performed, response, timeliness, checking-in, service reports, general compliance with contract specifications and technical requirements.
