About this file

This is a Phase I Submission Form for the Tsunami Coastal Analysis and Synthesis System (CASS) Request for Quotation (RFQ) 1305M326Q0019. The form collects general information from quoters and serves as the entire Phase I quotation submission; quoters must not submit Phase I information directly to the Government, and questions must be submitted via the Question Submission Form linked in the SAM.gov announcement.

Required Phase I submissions include: quoter name, address, Unique Entity Identifier (UEI), name of the representative authorized to obligate the Government, email, and phone number. Quoters must upload a signed SF 1449 form, signed SF 30s for each amendment issued during Phase I (combined into one file if multiple amendments exist), and completed Section K representations and certifications, or documentation that these have been completed in SAM. Failure to upload the signed SF 1449 or acknowledge all amendments may result in non-compliance determination. For Factor 1 (Experience), quoters must submit a PDF not exceeding six pages that uses graphics or visuals to summarize a specific company-developed platform software/application or development process demonstrating ability to perform CASS work. Relevant experience areas include High Performance Computing (HPC), cloud architecture, combining legacy code and applications into common software, seismological and hydrographical modeling, and short-term, high-impact iterative project management. The CASS contract requires creating a dashboard consolidating multiple disparate applications and backend processes written in different coding languages on Windows and Linux platforms. Quoters must indicate whether their company meets the small business size standard for NAICS code 541512 at submission time. Quoters selected for Phase II will elaborate on their Factor 1 example during the Factor 3 oral presentation.

Use this section to upload the signed SF 1449, a signed SF 30 for each amendment issued during the Phase I submission period, and the required certifications and representations. Failure to upload a signed SF 1449 may result in the Quoter's quotation being deemed non-compliant in accordance with RFQ addendum to FAR 52.212-1 paragraph L2.d.

If more than one amendment is issued during Phase I, the applicable SF 30s should be combined into one file and uploaded together. Failure to acknowledge every amendment may result in the Quoter's quotation being deemed non-compliant in accordance with RFQ addendum to FAR 52.212-1 paragraph L2.d.
