About this file
This is a pricing schedule (Schedule B) for a Request for Proposal issued by the State of Michigan for third-party administration of Flexible Spending Accounts under RFP No. 200000001275. The contract encompasses administration of Health Care FSAs, Dependent Care FSAs, and combined HC and DC FSAs, with a projected total of 13,112 participants across all enrollment categories. The contract term covers three plan years, with pricing to be submitted on a per-participant-per-month (PPPM) basis. Contractors are required to address how participant fees will be charged during termination periods, carryover periods, grace periods, and annual run-out periods.
Pricing must be submitted as separate PPPM administrative fees for each of the three plan years and each participant category (HC FSA with 11,455 participants, DC FSA with 385 participants, and HC & DC FSA with 1,272 participants), with calculations of total costs for each year and cumulative costs over the three-year period. The proposal must include all costs associated with service delivery, including one-time set-up charges and any additional or optional fees. Contractors are required to certify that pricing was developed independently and without consultation with other bidders. The schedule also requests disclosure of any implementation credits being offered for implementation services, communications, or third-party audits within the first two years of the contract.
