About this file
This is a Respondent Questionnaire attachment issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the Public Works Department for a Request for Quotation (RFQ) to supply Traffic Delineators. The questionnaire is designed to evaluate respondent qualifications, experience, capacity, and organizational structure. Respondents must provide contact information for a single point of contact and project management personnel, complete background information including legal business name, business entity designation, tax status, Federal Employer Identification Number, state of formation, years in business, and employee counts for the past three years. Additionally, respondents must document at least three relevant experiences from the past five years that demonstrate their ability to manage contracts of similar size and scope, with detailed information about each reference including the project name, client location, contact person details, dates of service, and description of services provided.
Respondents must disclose any prior contracts with the City of Detroit within the past five years, claims or lawsuits against the organization within the past ten years, and any terminated contracts. The questionnaire requires respondents to identify all proposed subcontractors, provide their legal business names and addresses, describe the goods or services they will supply, and verify that subcontractors are not on the City of Detroit's debarred and suspended suppliers list. Respondents must also demonstrate organizational capacity through submission of an organizational chart identifying key personnel, resumes for all key personnel, documentation of employee quantities that can be dedicated to the contract, and information regarding the experience level of listed employees. The document was last revised on December 2, 2025.
