About this file
This is an affidavit and compliance documentation template for Jackson County, Missouri's Request for Proposal No. 26-006. The document serves as the required certification and attestation form that bidders must submit with their proposal responses. The affidavit requires the bidder's authorized representative to swear under oath that no county officers, agents, or employees have a financial interest in the bid, that no county personnel would benefit from contract profits, and that the bidder is either in good standing on the county's tax rolls or had no taxable property as of December 31, 2025. Additionally, bidders must certify that they have not engaged in collusion or restraint of trade, are not on the General Services Administration's debarred and suspended parties list, are enrolled in a federal work authorization program, and do not knowingly employ unauthorized aliens. The document includes an acknowledgment of receipt section for addenda, with the question response deadline established as 5:00 p.m. CST on March 24, 2026, and provides space for bidders to document any exceptions to the scope of services.
The file also contains instructions for obtaining a Certificate of Compliance from Jackson County's Compliance Review Office, which is mandatory for all vendors doing business with the county and must be submitted with bid responses. The Certificate of Compliance application must be submitted five business days prior to the bid deadline to allow for processing and certifies that vendors meet tax compliance requirements and agree to Chapter 6 of the Jackson County Code regarding non-discrimination and equal employment opportunity. Failure to obtain and submit the Certificate of Compliance may result in bid rejection. Bidders are directed to contact the Compliance Review Office at compliance@jacksongov.org or visit the county's website for application procedures.
