About this file

This is an affidavit and compliance document package issued by Jackson County, Missouri for Request for Proposal (RFP) 25-054. The document serves as a standardized form requiring bidders to certify compliance with county regulations, tax obligations, and federal work authorization requirements. Bidders must submit a notarized affidavit confirming that no county officers, agents, or employees have financial interests in the bidder's proposal, that the bidder has not engaged in collusion or restraint of trade, and that the bidder is not listed on the General Services Administration's debarred or suspended parties list. The form includes sections for acknowledging receipt of addenda, which may be issued until the Question Response Deadline of 5:00 pm CST on January 13, 2026, and for noting any bidder exceptions to the scope of services. Respondents are required to check the Bonfire Portal regularly for any new addenda issued prior to the deadline.

All vendors responding to Jackson County solicitations must obtain a Certificate of Compliance from the Jackson County Compliance Review Office prior to bid submission, with applications due five business days before the bid response deadline. The Certificate of Compliance verifies that vendors are either duly listed and assessed on Jackson County tax rolls and not delinquent in taxes, or had no taxable property in the county as of December 31st of the previous year. Additionally, vendors must attest to compliance with Chapter 6 of the Jackson County Code regarding non-discriminatory practices and equal employment opportunity. Failure to submit a Certificate of Compliance may result in bid rejection. Further information and applications are available through the Jackson County Compliance Review Office at compliance@jacksongov.org or via the county's website.
