The RFP seeks staffing agencies to provide non-medical personal-care temporary support staff to assist students with moderate to severe cognitive, medical, physical, and/or behavioral disabilities. HCPSS intends to award contracts to multiple vendors at fixed-hourly rates for these positions. The specific contract response dates, due dates, bidder conferences, site visits, award dates, and contract terms are not detailed in this appendix document.

This appendix contains the mandatory certification language required by Federal Regulations Implementing Executive Order 12549 and applicable to multiple federal departments including Agriculture, Labor, Education, and Health and Human Services. Prospective offerors must certify that they and their principals are not debarred, suspended, or otherwise ineligible; have not been convicted of fraud or criminal offenses related to public transactions within three years; are not currently indicted for such offenses; and have not had public transactions terminated for cause or default within three years. False statements on this certification may result in bid rejection, award termination, or criminal prosecution.
