The Indian Health Service, Nashville Area Office intends to award at a minimum of 2single fixed-price, or at a maximum of 3 multiple award BPA in response to proposals to this RFQ.
This solicitation is restricted to 100% Small Business Concerns The solicitation documents, incorporated provisions, and clauses are those in effect through the Federal Acquisition Regulations (FAR). The associated North American Industry Classification System Code (NAICS) is 541370 surveying and mapping, and the small business size standard is $15 million.

C. Description/Specifications
The Indian Health Service, Nashville Area Office seeks to establish a BPA to award individual task orders. Except as otherwise specified herein, the Contractor(s) shall furnish the necessary personnel, materials, services, and do all things necessary to satisfactorily perform the work set forth below. IHS anticipates establishment of multiple BPAs as a result of this RFQ. Task Orders will be issued under the respective BPAs for project specific services.
C.1. STATEMENT OF WORK
a. Project Summary
The Contractor(s) will provide Surveying, Engineering, and Technician Support Services, Construction Inspection Support Services associated with various phases of the design and construction of sanitation facilities construction projects to the Northern District of the Nashville Area of the Indian Health Service, Office of Environmental Health and Engineering (OEHE), Division of Sanitation Facilities Construction (SFC).
The work consists of supporting water, wastewater, and solid waste sanitation facilities construction for the IHS Manlius Field Office, New York (IHS SFC office that serves Federally Recognized Tribes in New York)
Work to be accomplished by the contractor will be performed on federally recognized Tribal reservations and tribal sites primary in the states of New York and Virginia
The COR shall access the services provided under this contract by written directives to the Contractor(s) based on the hourly rates established in the contract.
Due to the highly variable nature/volume of services the Contractor(s) must be available, on short notice, to perform the required work; generally within 5 days.
Services under this BPA may include, but is not limited to:
1) Surveying Services
2) Engineering Technician Services
3) Construction Inspection Support Services
C.2. DESCRIPTION OF SERVICES
The Contractor(s) shall provide Surveying Services and Engineering Technician Support Services that includes: soil surveys, drafting services to support engineering design work, and Construction Inspection Support.
a. Place of Duty
Primary field-related duties are at tribal sites in New York and Virginia
b. Duty Hours
Duty hours may vary as appropriate for interaction with SFC staff, as directed by the COR. Typical duty hours are from 8:00am to 5:00pm.
c. Work Oversight
Work performed by the Contractor(s) shall be overseen by and under the direction of the Contracting Officer Representative (COR). The Government will provide the Contractor(s) written directives that may be in the form of an email that will include work to be completed, including timeframes and schedules for completion. The COR will be responsible for the monitoring and record keeping procedures used to ensure proper accounting for hours worked. These procedures must demonstrate through time and attendance logs, sign in/sign out sheets, or other appropriate records that services called for by the Contractor(s) have been rendered.
d. Travel
The Contractor(s) may be required to travel to reservations and assigned locations. The Contractor(s) shall be required to utilize the Contractor’s own vehicle(s) for travel. If the Contractor(s) intends to require more than one staff member on a trip, this must be approved in writing by the COR. Travel required and authorized by the Government to perform assigned tasks shall be reimbursed using the appropriate per diem and other travel expenses reimbursement rates established in the Federal Travel Regulation (FTR). Such reimbursement rates are adjusted from time to time. The current rates can be found at the following urls:

If the Contractor’s personnel incur expenses that exceed the allowable reimbursement rates, such excesses will not be reimbursed unless the Contractor(s) requests from the Contracting Officer with justification and is granted a waiver to the reimbursement rate limits prior to incurring the travel-related expenses. The Contractor(s) shall submit payment requests for travel after all travel for a particular project trip has been completed or monthly, as applicable.
All applicable paid receipts, including lodging, airfares, fuel, parking and other travel-related expenses are required for reimbursement at the rates authorized by Federal Travel Regulations (FTR). Requests for payment of travel expenses will be processed the same as normal payment requests. The Government will either include or add a line item to the applicable deliver order, as applicable, for travel reimbursement requirements. Such travel line items will be funded with a “Not-to-Exceed” (NTE) amount on the delivery order.
C.3. SCOPE OF WORK
Work shall be billed in two categories: Surveying Services and Engineering Technician Services, and Construction Inspection Support Services. All work files must be provided in original data format to the COR (e.g. Portable Document Files (PDF), spreadsheets, Word documents, AutoCAD files, point files, etc).
a. Surveying Services
i. Conduct geo-referenced surveys for proposed community water and wastewater systems to develop alignments
ii. Conduct geo-referenced surveys for proposed onsite water and wastewater systems
iii. Conduct geo-referenced surveys for record drawing development
iv. Download and process survey data to input into Computer Aided Drawing (CAD) software
b. Engineering Technician Services
i. Work includes reports and technical specifications, engineering evaluations and investigations, and design of sanitation facilities construction projects.
ii. Design and permitting of wastewater disposal facilities for single-family homes.
Services to include evaluation of site, site soils surveys, physical constraints, and environmental concerns. Designs shall include site boundary/topographic surveys, percolation tests, permit fees, and any other field investigations necessary to support https://www.gsa.gov/travel-resources http://www.gsa.gov/portal/content/100715 designs. Designs and permits shall be completed in accordance with governing jurisdiction (County Board of Health, City, Town, State, etc.).
iii. Draft construction plans, details, record drawings, composite system drawings and rights-of-way maps using AutoCAD Civil 3D 2015 (or later). All work must comply with IHS drafting standards.
c. Construction Inspection Support
i. Provide IHS inspection support on sanitation facilities construction projects. Take notes and photos, and record quantities and measurements. Communicate regularly with IHS Construction COR and Engineer to ensure that all work is performed in accordance with the approved plans and specifications.
ii. The Contractor awarded work to do Surveying and Engineer Technician Services on a project will be the same Contractor that provides inspection support services for the same project if IHS is unable to provide inspection.
C.4. REQURED QUALIFICATIONS
The Contractor(s) shall possess the following minimum qualifications.
Surveying- The Contractor shall be a licensed Professional Land Surveyor (or shall have a Professional Land Surveyor on staff) with at least 10 years of experience and meet State requirements.
Preparation of on-site wastewater designs qualifications: The Contractor(s) shall be able to prepare layouts for individual on-site water and wastewater facilities for review. This shall include downloading electronic survey data from a data collector and/or hand-written field notes and producing a site plan. Contractor(s) shall be able to prepare final design of the necessary facilities, following all applicable codes and standards.
Drafting Capabilities - Must have knowledge and experience in the design of water and wastewater facilities and other civil engineering projects using Computer Aided Design (CAD). Must have CAD knowledge and experience related to plan set structure, function and geometrics and skill in technical CAD drafting using AutoCAD. Contractor(s) must be proficient in AutoCAD Civil 3D 2015 (or later) design software including Plan Production Tools and Sheet Manager for plan-set development and plotting purposes. Contractor(s) must have the ability to incorporate new surfaces from Triangular Irregular Network (TIN) models, Digital Terrain Models (DTMs), orthoquads, etc. into existing design model files and to integrate separate images, files and data sets. Contractor(s) must have the ability to utilize Civil 3D functionality including pipes, alignments, grading, profiles, blocks, styles, etc.
Written Communication – Must have strong written communication skills with experience related to drafting guidelines and technical provisions and compiling final reports as related to sanitation facilities construction projects.
Construction Inspection Support- The Contractor shall have at least 10 years of experience doing construction inspection.
C.5. PREFORMANCE
Each task order shall take no longer than 5 days to complete. Each task order shall serve as the Notice to Proceed (NTP) for that individual task.
D. BPA Terms and Conditions
D.1. DESCRIPTION OF AGREEMENT
The Offeror shall furnish services in accordance to the Statement of Work if and when requested by the Contracting officer or the authorized representative appointed by the contracting office during the specified period of performance and within the agreed amount.
D.2. EXTENT OF OBLIGATION
The Government is obligated only to the extent of authorized purchases actually made under the BPA.
D.3. PURCHASE LIMITATIONS
The Dollar limit for this agreement is as follows
Minimum amount per task order: $100.00 Maximum amount per task order: $100,000.00 Maximum total amount: $250,000.00
D.4. INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THE BPA
The contracting officer will provide the contractor a list of individuals identified either by title of position or by name of individual, or organizational component authorized to place orders under this agreement. If a call is not made by an individual listed on the authorized list, the call will be at the sole responsibility of the contractor.
D.5. DELIVERY TICKET
All services rendered under this agreement shall contain the following information;

d. Order number/site name
e. Itemized list of services furnished, and extended pricing
f. Date of services rendered, and acceptance
D.6. INVOICES
The Contractor shall submit an invoice monthly for all services rendered during the billing period, including all delivery tickets covered therein.

For invoices submitted by email: the subject line of your email invoice submission shall contain the contractor name, contract/order number, and invoice number (e.g. Company Name, Contract HHSIXXXXX, Invoice #123). The Contractor shall send one email per contract/order after completion.
The email may have multiple invoices for the contract. Invoices must be in one of the following formats:
PDF, TIFF, or Word. No Excel formats will be accepted. The electronic file cannot contain multiple invoices; example, 10 invoices requires 10 separate files (PDF, TIFF, or Word).
D.7. MODIFICATIONS
The Contracting Officer (CO) is the only person authorized to approve changes or modify any of the requirements contained in this contract. In the event the contractor effects any such change at the direction of any other person other than the CO, the change will be considered to have been made without authority and no adjustment will be made in the contract costs to cover any increase incurred as a result thereof.
Contractual problems, of any nature, that may arise during the life of this contract must be handled in conformance with very specific public laws and regulations (i.e., Federal Acquisition Regulation). Only the CO is authorized to formally resolve such problems. The Contractor must bring all such contractual problems to the immediate attention of the CO. Requests for modifications of this contract must be sent to the CO with a copy sent to the COR as designated in the COR appointment letter.
D.8. GOVERNMENT POINTS OF CONTACT
Contracting Officer:
All contract administration will be effected by the Contracting Officer. Communications pertaining to contractual administrative matters will be addressed to the Contracting Officer. No changes in or deviation from the scope of work shall be effected without a written modification to the contract executed by the Contracting Officer authorizing such changes.
Contracting Officer Representative (COR):
A COR will be designated on authority of the Contracting Officer to monitor and coordinate all technical aspects and assist in the administration of the contract. All contacts with all agencies of the Government and interfacing with other contractors required in the performance of this contract will be accomplished only through the direction and with the coordination of the COR. A letter of designation will be issued to the COR with a copy supplied to the Contractor, stating the responsibilities and limitations of the COR.
E. Applicable Clauses
52.236-5 Material and Workmanship. (APR 1984)
(a) All equipment, material, and articles incorporated into the work covered by this contract shall be new and of the most suitable grade for the purpose intended, unless otherwise specifically provided in this contract. References in the specifications to equipment, material, articles, or patented processes by trade name, make, or catalog number, shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may, at its option, use any equipment, material, article, or process that, in the judgment of the Contracting Officer, is equal to that named in the specifications, unless otherwise specifically provided in this contract.
(b) The Contractor shall obtain the Contracting Officer's approval of the machinery and mechanical and other equipment to be incorporated into the work. When requesting approval, the Contractor shall furnish to the Contracting Officer the name of the manufacturer, the model number, and other information concerning the performance, capacity, nature, and rating of the machinery and mechanical and other equipment. When required by this contract or by the Contracting Officer, the Contractor shall also obtain the Contracting Officer's approval of the material or articles which the Contractor contemplates incorporating into the work. When requesting approval, the Contractor shall provide full information concerning the material or articles. When directed to do so, the Contractor shall submit samples for approval at the Contractor's expense, with all shipping charges prepaid. Machinery, equipment, material, and articles that do not have the required approval shall be installed or used at the risk of subsequent rejection.
(c) All work under this contract shall be performed in a skillful and workmanlike manner. The Contracting Officer may require, in writing, that the Contractor remove from the work any employee the Contracting Officer deems incompetent, careless, or otherwise objectionable.

52.246-4 Inspection of Services-Fixed-Price (Aug 1996)
(a) Definition.“Services,” as used in this clause, includes services performed, workmanship, and material furnished or utilized in the performance of services.
(b) The Contractor shall provide and maintain an inspection system acceptable to the Government covering the services under this contract. Complete records of all inspection work performed by the Contractor shall be maintained and made available to the Government during contract performance and for as long afterwards as the contract requires.
(c) The Government has the right to inspect and test all services called for by the contract, to the extent practicable at all times and places during the term of the contract. The Government shall perform inspections and tests in a manner that will not unduly delay the work.
(d) If the Government performs inspections or tests on the premises of the Contractor or a subcontractor, the Contractor shall furnish, and shall require subcontractors to furnish, at no increase in contract price, all reasonable facilities and assistance for the safe and convenient performance of these duties.
(e) If any of the services do not conform with contract requirements, the Government may require the Contractor to perform the services again in conformity with contract requirements, at no increase in contract amount. When the defects in services cannot be corrected by reperformance, the Government may-
(1) Require the Contractor to take necessary action to ensure that future performance conforms to contract requirements; and
(2) Reduce the contract price to reflect the reduced value of the services performed.
(f) If the Contractor fails to promptly perform the services again or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may-
(1) By contract or otherwise, perform the services and charge to the Contractor any cost incurred by the Government that is directly related to the performance of such service; or
(2) Terminate the contract for default.

(a) As required by sections 744 and 745 of Division E of the Consolidated and Further Continuing Appropriations Act, 2015 (Pub. L. 113-235), and similar provisions, if contained in subsequent appropriations acts, the Government will not enter into a contract with any corporation that-

52.219-1 Small Business Program Representations. (OCT 2014)
(a) Definitions. As used in this provision- Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business concern eligible under the WOSB Program.
Service-disabled veteran-owned small business concern-
(1) Means a small business concern-
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.
(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
Small business concern means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and the size standard in paragraph (b) of this provision.
Small disadvantaged business concern, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that-
(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by-
(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States, and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and

(2) The management and daily business operations of which are controlled by one or more veterans.
Women-owned small business concern means a small business concern-
(1) That is at least 51 percent owned by one or more women; or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
(2) Whose management and daily business operations are controlled by one or more women.

H. Evaluation Factors
The Government plans to award a single fixed price contract in response to this solicitation. Except when it is determined in accordance with FAR section 17.206(b) not to be in the Government’s best interests, the Government evaluates offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options does not obligate the Government to exercise the option(s).
The Government intends to conduct the source selection in accordance with the competitive negotiated source selection procedures and Best Value Continuum contained in FAR Part 15. All evaluation factors other than cost or price, when combined, approximately equal to cost or price.
H.1. Basis for Award
In accordance with FAR Clause 52.215-1, Instructions to Offerors - Competitive Acquisition, the Government intends to award a contract resulting from this solicitation to the responsible offeror whose proposal represents the best value after evaluation in accordance with the factors, subfactors, and elements specified in the solicitation. The Government will evaluate proposals and award a contract without discussion with offerors (except for clarifications as described in FAR Subpart 15.3). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a technical and cost and price standpoint. The Government reserves the right to conduct discussions if the contracting officer later determines them to be necessary.
Award decisions will be based on the Government’s evaluation of each offeror’s complete proposal against the evaluation criteria identified in Section Below
H.2. Evaluation Criteria
Factor 1 – Past Performance
The Government will assess the offeror’s capability to perform under the solicitation by evaluating the offeror’s past performance as a prime contractor on previous contracts similar in size, scope and complexity. The evaluation will focus on the offeror’s technical understanding and technical capability as demonstrated by the results achieved in the completion of actual contracts and task orders similar in size, scope, and complexity.
Factor II – Technical Proposal
The Government will the assess offeror’s suitability and technical ability in relation to similar requirements anticipated in the contract; and the soundness, completeness, and adequacy of the understanding of key personnel.
In response to the solicitation, the Government will evaluate the offeror’s proposed technical understanding to be able to meet the mission requirements and performance objectives.
In response to the solicitation, the Government will evaluate the offeror’s key personnel’s qualifications and certifications, to establish the contractors suitability in relation to the contract requirements
In response to the solicitation, the government will evaluate the work history of key personnel to establish the relevance of the contractors work history and ability to meet the scope of the contract requirements
