Statement of Work for Maintenance and Repair Plan for 75 Indus 4601-11 Microfilm Readers

1.0 SCOPE

Objective:

1.1 The Social Security Administration’s Office of Earnings and International Operations (OEIO) is seeking to obtain a maintenance and repair plan that covers maintenance and repair services, parts and labor for 75 government-owned Indus Microfilm Readers, Model Number 4601-11.

Background:

1.2 OEIO maintains and houses a variety of microfilm used for establishing and correcting workers’ earnings records, furnishing earnings statements, and resolving individual and employer earnings problems. This is a major workload for OEIO, which involves using microfilm readers. Since the employees are required to use microfilm readers for part of workload processing, it is imperative that each microfilm reader is operating properly and available for use at all times.

1.3 The Indus microfilm readers are located in the OEIO Library Room on the Lower Level, C-5-C South Tower at 6100 Wabash Avenue, Baltimore, Maryland 21215 and several other locations (e.g., employee workstations) within the complex.

1.4 OEIO owns 75 microfilm readers; however, approximately eight of the readers have not been located, but are believed to be on the premises. In the event any of the misplaced equipment is located, that equipment will be maintained and repaired under this agreement at no additional cost to the Government.

1.5 Fifty (50) of the microfilm readers were manufactured in 2014 and twenty-five (25) in 2015. An equipment listing including the serial numbers for the 75 microfilm readers is attached.

2.2 The contractor shall provide onsite service within 4 hours to one day of the service call.

2.5 The contactor shall furnish all labor, parts and components, and related material necessary for the maintenance and repair of the equipment covered by the agreement.

2.6 The contractor shall provide documentation that it is an authorized reseller for Indus micro film readers for obtaining necessary parts and components to provide the maintenance and repair services. The details, including serial numbers, for the microfilm readers are specified on the attached equipment listing. The contractor must be able to perform the maintenance and repair services for the microfilm readers 30 days after award with a period of performance for 12 months.
