About this file

This is an Exhibit B Price Schedule for Sleep Apnea Diagnostic & Treatment Services (Contract 26PSX0134) issued by the State of Connecticut. The schedule establishes unit pricing for a comprehensive range of pulmonary consultation services delivered via telehealth, including sleep apnea testing, diagnosis, clinical recommendations, and equipment provision. The scope encompasses clinical assessments (initial and follow-up telehealth evaluations), portable home sleep apnea testing (HSAT) with repeat testing provisions, diagnostic reporting by registered polysomnographic technologists and board-certified sleep physicians, pulmonary consultation services, positive airway pressure (PAP) therapy equipment available for purchase, lease, or rental in three modalities (CPAP, AutoPAP, and BiPAP), replacement supplies and accessories, training sessions, 24/7 technical and clinical support, utilization and compliance reporting, and optional services. The contract specifies that all implementation services required under Exhibit A, including implementation planning, contract management, equipment deployment, testing, validation, and go-live support, shall be included in the proposed unit prices and not separately invoiced.

Bidders must enter pricing only in designated yellow-highlighted cells, with all prices including labor, supervision, administration, travel, delivery, setup, training, software, documentation, warranty support, and other necessary performance costs unless expressly stated otherwise. Equipment offerings must identify manufacturer, model, warranty period, and replacement response time, while leased equipment pricing must encompass maintenance, repairs, calibration, updates, replacement parts, technical support, and substitute equipment. Repeat testing caused by contractor error, equipment malfunction, lost data, or invalid results shall be provided at no charge. The contract specifies a 45-day payment term with prompt payment discount availability. Pricing adjustments are permitted only during 30-day periods immediately preceding annual contract anniversary dates and must be justified by Bureau of Labor Statistics Producer Price Index data with supporting documentation; DAS retains sole discretion to approve or disapprove adjustments in whole or part. Approved adjustments become effective ten days after approval date, and the contractor must honor purchase orders issued prior to adjustment effectiveness at the then-current price.
