About this file

This is a Bidder's Checklist for State of California Solicitation Number 26CRY001-I2, issued by the California Department of Veterans Affairs for Mobile Diagnostic Radiology and Ultrasound Services at the Veterans Home of California in Yountville. The checklist specifies the required documentation that bidders must submit with their proposals to ensure compliance with the invitation for bid requirements. While specific response dates, due dates, site visits, bidder's meetings, and award dates are not detailed in this checklist document, bidders are instructed to return the completed checklist along with their bid submissions.

Bidders must submit a comprehensive set of required documents including a Bidder Certification, Bid Rate Sheet, signed Payee Data Record (STD 204), Bidder Declaration (GSPD-05-105), Non-Collusion Declaration, and Contractor Certification Clauses (CCC-4/2017). Additional certifications and declarations are conditionally required depending on applicability, including Commercially Useful Function (CUF) Certification, Disabled Veteran Business Enterprise (DVBE) Declarations (DGS PD 843), Iran sanctions compliance, and Darfur Contracting Act certification. Bidders must also provide valid California city or county business licensing documentation or equivalent proof of business standing. The checklist emphasizes that company names must be identical across all submitted documents and references the California Department of General Services website for standard contract language resources. A sample invoice format is also required as part of the submission package.
