This is an application response form from the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health (DPH) for RFR #261034 titled "Family Supportive Living." The form provides detailed instructions for bidders to submit proposals for providing family recovery support services, including case management within a recovery-oriented and trauma-informed living environment. The program focuses on supporting parents in early recovery from substance use disorder (SUD), with particular emphasis on services for pregnant and postpartum mothers, their infants, and DCF-involved families. Key requirements include providing parenting support, medication for opioid use disorder (MOUD) services, childcare, developmental services, and community-based support linkages.
Bidders must demonstrate a minimum 1% Supplier Diversity Program (SDP) commitment through partnerships with SDO-certified diverse businesses or non-profits. The application requires detailed budget submissions, including start-up costs and capital purchases, with capital items needing specific DPH approval and purchase within a 3-month start-up period. Providers must participate in local health coalitions and demonstrate capacity for culturally competent and linguistically appropriate services. The program aims to transition families to self-sufficiency within one to two years through housing and employment support services.
