About this file

This is an Application Response Form from the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health for RFR #260734 regarding Family Residential Treatment Services. The form requires bidders to submit detailed responses through COMMBUYS covering organizational structure, program implementation, staffing plans, and service delivery approaches for residential substance abuse treatment services for families. Key requirements include the ability to serve pregnant women, provide medication-assisted treatment, deliver culturally competent care, and ensure program entry within 7 days of receiving intake materials.

The application requires bidders to demonstrate a meaningful financial commitment through the Supplier Diversity Program (SDP), aiming to direct at least 1% of Commonwealth contract revenue toward diverse business enterprises or non-profit organizations. Bidders must submit detailed program and capital budgets, with capital items requiring specific DPH approval prior to purchase within the 3-month startup period.
