About this file
This document is an application form for the Massachusetts Breast and Cervical Cancer Screening Services (MBCCP) Request for Response (RFR) #260134. The MBCCP is seeking applications from eligible healthcare organizations to provide breast and cervical cancer screening and diagnostic services. Eligible organizations include community health centers, accountable care organizations, hospitals, health systems, mammography facilities, gynecology practices, and mobile clinics. Applicants must have staff capable of performing data entry in REDCap, agree to being listed on the MBCCP webpage and other promotional materials, and comply with a set of detailed program affirmations.
The affirmations require applicants to assess patient eligibility, obtain informed consent, conduct risk assessments, provide appropriate screening and diagnostic tests, ensure timely follow-up, refer patients to treatment programs, prioritize screening for underserved populations, maintain required licenses and certifications, use the MBCCP as a payer of last resort, seek reimbursement only for authorized services, provide services at no cost to patients, and submit monthly clinical, service, and billing data to the MDPH REDCap system.
