About this file

This is a Cost Worksheet for Invitation for Bid (IFB) 26-10041 issued by the California Department of Public Health (CDPH). The solicitation seeks qualified contractors to provide respirator medical clearance evaluations and fit testing services to CDPH staff throughout the state, including employees, contractors, Federal assignees, trainees, and EIS officers. Medical evaluations and fit testing procedures must comply with Cal/OSHA CCR Title 8 Section 5144 and ANSI standard Z88.10-2010. The cost worksheet identifies three service categories: Respiratory Fit Tests, Medical Evaluations, and Spirometry Tests, with per-test rates of $2,300, $2,300, and $300 respectively. The total cost will be calculated based on the estimated number of tests performed, with the grand total amount to be quoted serving as the final contract rate.

The allocated budget for this IFB shall not exceed $350,000.00, and any quotes exceeding this amount will be deemed non-responsive and ineligible for consideration. The contract will be awarded to the lowest responsible bidder on a time and material basis in accordance with the State Contracting Manual Volume 1, Section 7.30. Contractors are advised that all quoted rates must be fully loaded to include operating expenses, labor costs, transportation, mileage, per diem, supplies, equipment, overhead, inflation adjustments, profit margin, and taxes. CDPH makes no guarantee that all allocated funds will be expended during the contract term; reimbursement will be limited to actual services performed and invoiced. Contractors submitting cost worksheets certify that their quoted prices are accurate and will remain in effect throughout the agreement duration and acknowledge that their quote response will become a public document open to public inspection.
