About this file

This document is a Darfur Contracting Act Certification form issued by the California Department of General Services (DGS) Procurement Division. The form applies to companies that have conducted business activities outside the United States within the previous three years and are seeking to bid on or submit a proposal for a State of California contract. The document outlines two certification options: Option #1 requires companies to certify they are not a "scrutinized company" under Public Contract Code Section 10476, while Option #2 allows scrutinized companies to submit a bid if they have obtained written permission from the Department of General Services.

The certification form requires companies to provide their vendor name, Federal ID Number, and an authorized signature certifying their status. Companies with no business activities outside the United States in the past three years are not required to complete the form. For scrutinized companies seeking to bid, they must include a copy of written permission from DGS with their proposal, demonstrating that their participation is in the best interests of the state on a case-by-case basis. The form is designed to ensure compliance with Public Contract Code Sections 10475-10481 and must be signed under penalty of perjury.
