PERFORMANCE WORK STATEMENT

Performance Work Statement for

Cadaver and Cadaver Services for the Advance Surgical Skills for Exposure in Trauma (ASSET+) and Combat Orthopedic Trauma Surgery (COTS) Course

1.0 Scope. 2nd Medical Battalion (2D MED BN) requires cadavers and cadaver support services for its

Advance Surgical Skills for Exposure in Trauma (ASSET+) and Combat Orthopedic Trauma Surgery (COTS) training. The contractor will be required to work closely with 2D MED BN in the delivery, preparation, installation, and disposition of the contracted items and services.

2.0 Applicable Certifications.

2.1 Accredited by the American Association of Tissue Banks (AATB)

2.2 Licensed by a state Board of Health

2.3 Accreditation from competing organizations may be considered.

2.4 Interested vendors in non-licensing states may be considered for award.

Measures

4.0 General Requirement: The contractor shall provide cadaver provision, refrigerated transport and custodial services, preparation and installation of the cadaver subjects, biomedical waste collection and disposal during the cadaver laboratory training, and cadaver disposition upon completion of the event.

4.1 Contractor shall provide all cadaver material, and cadaver preparation material and tools.

4.2 Contractor shall provide all biomedical waste collection and storage materials.

4.3 Contractor shall provide all biomedical waste disposal services. There will be no disposal available on the installation.

4.4 Contractor shall provide all services related to the disposition of the cadavers upon ASSET+/COTS course completion.

5.0 Specific Requirements.

5.1 Contractor shall provide refrigerated transport for six (6) cadavers from the vendor point of origin to Basic

Skills Training School (BSTS) Tactical Combat Casualty Care (TCCC) classroom Camp Lejeune Trail, Camp Lejeune, NC 28547.

5.2 Contractor shall provide refrigerated transport for three (3) cadavers from the vendor point of origin to Basic Skills Training School (BSTS) Tactical Combat Casualty Care (TCCC) classroom Camp Lejeune Trail, Camp Lejeune, NC 28547 on 27AUG2024.

5.3 Contractor shall provide refrigerated transport for three (3) cadavers from the vendor point of origin to Basic Skills Training School (BSTS) Tactical Combat Casualty Care (TCCC) classroom Camp Lejeune Trail, Camp Lejeune, NC 28547 on 28AUG2024.

5.3 Contractor shall maintain refrigerated custody of cadavers until delivery for preparation and installation at Basic Skills Training School (BSTS) Tactical Combat Casualty Care (TCCC) classroom Camp Lejeune Trail, Camp Lejeune, NC 28547 no later than: 27AUG2024 for (3) cadavers and 28AUG2024 for (3) cadavers.

5.4 Contractor shall remain on site at Basic Skills Training School (BSTS) Tactical Combat Casualty Care (TCCC) classroom Camp Lejeune Trail, Camp Lejeune, NC 28547 for biomedical waste collection and storage services on 27AUG2024 for the first (3) cadavers and on 28AUG2024 for the last (3) cadavers.

5.5 Contractor shall provide disposition services for the expended cadavers in accordance with applicable federal, state and local guidelines. There will be no disposal available on the installation.

6.0 Contractor Qualifications Contractor must be able to provide evidence of certification to transport, handle, prepare, and disposition cadaver subjects.

6.1 Contractor must be able to provide evidence of certification to handle and dispose of biomedical waste materials.

7.0 Cadaver Requirements. Contractor shall provide six (6) human full body cadavers in accordance with the following:

7.1 Ectomorph or mesomorph male or female.

7.2 Age 21-85; 60-78 inches tall; 100-220 pounds.

7.3 No major constriction, no communicable diseases, no major organs missing, and no major trauma.

7.4 All cadavers shall be delivered without prosecting/dissecting.

7.5 All cadavers shall be fresh, frozen.

7.6 Cadavers must be delivered in a refrigerated truck and thawed out prior to being used on the 27th and 28th of

August.

8.0 Government Provided Support

8.1 Unit specific requirements. Unit specific requirements will be provided by the unit (ASSET+/COTS training supplies, surgical/medical equipment, coverings and protection for beds/litters).

8.2 Tents/Venue. Unit will provide laboratory space and associated utilities during the performance of the

ASSET+/COTS course.

8.2.1 Venue specifically will be in the BSTS TCCC Classroom.

8.2.2 Access will be controlled via an access roster and guarded entrance and unauthorized access signs prohibited posted.

8.2.3 Venue will have sanitary facilities. The venue will abide by OSHA rules and regulations with procedures for safety (biological, chemical, fire egress) posted.

8.2.4 Photography will be allowed in the venue where cadavers are present; however, photography of the cadavers will be prohibited. Photography will be allowed in other areas of the training (classroom area and dry lab (mannequin) area).

9.0 Government Point of Contact: The Government points of contact are not warranted Contracting Officers and do not have the authority to take any action, either directly or indirectly, that would change the pricing, quantity, place of performance, delivery schedule, or any other terms and conditions of the contract.

10.0 Period of Performance. The period of performance for available training is 27-29 AUG 2024.

12.0 Contractor’s Unauthorized Work Performance. The Contractor shall not perform work that deviates from contract requirements and specifications. If the Contractor deviates from contract requirements and specifications without approval of the CONTRACT ADMINISTRATOR/OFFICER, such deviation shall be at the risk of the Contractor and any cost related thereto shall be borne by the Contractor.

-Contractors must be registered in the Defense Biometrics Identification System (DBIDS). It is the Contractor’s responsibility to ensure all their employees and/or subcontractors are properly registered within DBIDS.

-DBIDS credentials will be valid for the duration of your contract, not to exceed three years; however, re-vetting of individuals' information will occur each year.

-Contractors requiring access for over 60 days will receive a credential valid for up to one year.

-Contractors requiring access for less than 60 days will receive a temporary paper pass.

-Contractor Common Access Card (CAC) Eligibility: Contractor personnel authorized CACs are exempt from DBIDS enrollment.

-If you have questions regarding base access, Contractor CACs, or DBIDS requirements, contact the Contractor Vetting Office (CVO). The CVO is located in Bldg. 818 adjacent to the Camp Lejeune Main Gate on Holcomb Blvd, just off NC Highway 24. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. 252.223-7001 HAZARD WARNING LABELS (DEC 1991)

(a) "Hazardous material," as used in this clause, is defined in the Hazardous Material Identification and Material Safety Data clause of this contract.

(b) The Contractor shall label the item package (unit container) of any hazardous material to be delivered under this contract in accordance with the Hazard Communication Standard (29 CFR 1910.1200 et seq). The Standard requires that the hazard warning label conform to the requirements of the standard unless the material is otherwise subject to the labeling requirements of one of the following statutes:

(1) Federal Insecticide, Fungicide and Rodenticide Act;

(2) Federal Food, Drug and Cosmetics Act;

(3) Consumer Product Safety Act;

(4) Federal Hazardous Substances Act; or

(5) Federal Alcohol Administration Act.

(c) The Offeror shall list which hazardous material listed in the Hazardous Material Identification and Material Safety Data clause of this contract will be labeled in accordance with one of the Acts in paragraphs (b)(1) through

(5) of this clause instead of the Hazard Communication Standard. Any hazardous material not listed will be interpreted to mean that a label is required in accordance with the Hazard Communication Standard.

MATERIAL (If None, Insert "None.") ACT

(d) The apparently successful Offeror agrees to submit, before award, a copy of the hazard warning label for all hazardous materials not listed in paragraph (c) of this clause. The Offeror shall submit the label with the Material Safety Data Sheet being furnished under the Hazardous Material Identification and Material Safety Data clause of this contract.

(e) The Contractor shall also comply with MIL-STD-129, Marking for Shipment and Storage (including revisions adopted during the term of this contract).

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (JAN 2023)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

“Payment request” and “receiving report” are defined in the clause at 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(b) Electronic invoicing. The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as authorized by Defense Federal Acquisition Regulation Supplement (DFARS) 252.232- 7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.sam.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order:

(1) Document type. The Contractor shall submit payment requests using the following document type(s):

(i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher.

(ii) For fixed price line items—

(A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting Officer.

(B) For services that do not require shipment of a deliverable, submit either the Invoice 2in1, which meets the requirements for the invoice and receiving report, or the applicable invoice and receiving report, as specified by the Contracting Officer.

(iii) For customary progress payments based on costs incurred, submit a progress payment request.
