SCOPE OF WORK
Women’s Clinic Renovation Project
Project # OK4CAS07H6 Revised August 12, 2016
I. Objective:
The objective of this project is to remodel multiple rooms in the Women’s Clinic area at the US PHS Indian Hospital located in Claremore, Oklahoma. The hospital is a 110,000+ ft2 hospital building that consists of outpatient services such as OB/GYN, Behavioral Health, Pediatrics, Medical/Urgent Clinics, Contract Health, Dental, WIC, Pharmacy, Diabetes Education, and Public Health, as well as in-patient services that includes ER, OB, in-patient, OR, and ICU.
II. Background:
A. Women’s Clinic:
The Women’s Clinic is in the original Administration area of the hospital built circa 1977.
The area is not designed for out-patient use. Renovation is needed to provide additional space for out-patient services and offices.
1. The existing breakroom 181 is an old janitor’s closet. This room is too small for breakroom use and will be expanded into a small office for the Benefit Coordinator by using floor space in room 183.
2. The existing Benefit Coordinator office in room 172 is an old exam room and needs to have the sink reinstalled and used as an exam room.
3. The current Women’s Clinic Waiting Room 153 is under-utilized with waiting mostly occurring in the hallway seating. This area will be converted into an employee break room for the Women’s Clinic and for a small Administrative Office.
4. The nursing staffing location in the large open area in room 178 requires staff facing the wall and leaving computers exposed to patients and other staff, creating a HIPPA issue with privacy concerns. In order to correct this problem, additional room will be taken from exam rooms 175 & 176 in order to provide a proper nurses’ station in the center of the large open room 178.
5. Room 171 does not have a sink currently installed in the room in order to convert the room into an exam room.
6. The existing Surgery Clinic layout is shown in the following attachments:
A. Current Women’s Clinic Layout
III. Scope of Work:

2. Contractor shall provide all labor, materials, supplies, tools and equipment, transportation, supervision and all other necessary items for remodeling the Surgery Clinic and Women’s Clinic.
