Respondents must certify compliance with 17 mandatory criteria questions and complete the PUR 7801 Vendor Certification Form and related attestation documents to be deemed responsive. The certification requirements address authorization to respond, conflicts of interest disclosures, vendor list status, E-Verify registration and compliance, Florida Department of State registration, boycott of Israel certifications, and Sudan and Iran activities restrictions. Additionally, respondents must confirm they administer at least one employer retirement program with over $3 billion in assets and at least 45,000 accounts, agree to collect explicit participant fees and remit administrative fees to the Department, provide an approved variety of investment products including equity, bond, fixed income, target date funds, low-cost index funds, and brokerage services, employ a minimum of 15 full-time equivalent account representatives dedicated to the Florida plan, utilize an appropriate recordkeeper system with daily aggregate reporting capabilities, establish secure data transmission interfaces, and offer advisory or managed account products. Failure to provide complete and accurate certifications for all questions will result in the respondent being deemed non-responsive, and false information discovered before contract award will result in deemed non-responsiveness, while false information discovered after award may result in contract termination and liability for re-procurement costs.
The certification form serves as a prerequisite to substantive proposal evaluation and requires authorized representatives to sign under penalty of perjury. Respondents must complete supplemental attestation forms including PUR 1355 (Foreign Country of Concern Attestation), PUR 1808 (Common Carrier or Contracted Carrier Attestation if applicable), and a labor coercion attestation pursuant to section 787.06, Florida Statutes. Annual resubmissions of attestation forms will not require written contract amendments. The form emphasizes that the Department will rely on the accuracy of provided representations in making its award decision and reserves enforcement remedies for misrepresentations discovered at any stage of the procurement process.
