Rev. (11/2018)

Addendum #1

To: Prospective Bidders

Reference: IFB #24-64044-000

Please see the following questions regarding the IFB (DSH responses in yellow):

1. In the IFB, Attachment 15 is not included within the document as referenced on the Cal Procure Event Details page. Can you provide Attachment 15?

Attachment 15 (Sample Agreement, STD. 213 & Exhibits) begins on page 34 of the IFB and continues through page 69.

2. Staff asked if we could recess the grinder equipment into the existing concrete floor roughly 4”. Yes, we can do that assuming the existing floor thickness is at least 8” thick. Please confirm if this is how you would like the grinder equipment installed.

Yes, DSH shall require the auger to be recessed a minimum of 4”.

3. Staff mentioned that the electrical terminations and new wiring-conduit to the panel and equipment motors could be provided by DSH Electricians. Please confirm if DSH will provide or if we should cover the cost of an electrician for that kind of ‘licensed code’ work.

DSH shall require the contractor to cover the cost of any required electrical work (sub-contracting is allowed, if needed).

4. In the IFB, Under Attachment 5 – Bidder’s Rate Sheet (page 26 of 69). Please confirm this ‘Parts Allowance’ covers any material we need and will be invoiced against that allowance.

‘Parts Allowance’ covers all materials required by the contractor to provide the requested services. Contractor invoices for parts shall be invoiced against the http://www.dsh.ca.gov/

24511 W. Jayne Avenue provided $10,000.00 allowance and are subject to review and approval by the DSH Contract Manager.

5. For documentation, we have asked for all equipment O&M’s and any detail equipment / panel drawings that were provided.

DSH shall provide the contractor with all relevant plans, drawings, and manufacturer O&M’s.
