About this file
This document is an addendum to RFP 24-25-040 "NFPA 1582 Physicals" issued by the City of Charlottesville, Virginia. The addendum clarifies questions regarding the onsite review, virtual meetings for out-of-state companies, RFP identification on the envelope, required signed documents, pricing for optional tests, the core program of required tests, the number of female employees, the desired testing timeframe, shift schedules, the capacity for participants per day, and whether blood draws require a separate schedule. The RFP is for the provision of NFPA 1582 physicals for the Charlottesville Fire Department.
The addendum indicates that the core program should include all elements required by NFPA 1582, and pricing should be provided as a lump sum. The department currently has two female employees, and the testing is desired to be completed prior to the end of 2024, with the flexibility to schedule it at any time going forward. The department can accommodate up to 25 participants per day, and all testing can be completed within one week. Blood draws do not require a separate schedule, and the department has an onsite treadmill that can be utilized for the stress tests.

• On the Sample Tests and Screenings list there are several items that have Option in the title such as the “Drug Screening Sample Test Options”. Are you looking for each of these to be priced out and all of them included in the lump sum price for services?
Am I to list a per unit price on each item listed in the Sample Tests and Screenings document or do you want an extended price for the entire department. Keeping in mind that some test (Heavy Metals, etc.) may not be required for all members.
Tests that are required for all employees should be included in the lump sum price. Any additional tests required by position or for other reasons should be priced individually.
