About this file
This is Addendum #1 to IFB #24-10038-000 issued by the Department of State Hospitals - Atascadero (DSH-A) for Courier Services for STAT Labs. The addendum clarifies that services will be needed on a daily basis, and bids must be submitted via mail or hand delivery (by appointment only), with email submissions not permitted. The contractor must provide drivers with valid licenses and HIPAA Training Certification as part of the Minimum Qualifications Response, with additional documentation available upon request after contract award.
The addendum specifies that fuel costs must be included in the bid amount, and order numbers will suffice for tracking purposes with no scanning required. DSH-A will not provide dry ice for transportation but will supply ice packs when necessary. The addendum also clarifies that signature pages from organized training sessions are acceptable in lieu of certificates for Blood Borne Pathogen and HIPAA training requirements.

Answer #5 Bidders are required to provide all the items listed in Attachment 3 – Minimum Qualifications Response (located on page 18 of 65) with their bid. Including, but not limited to, valid driver’s license and HIPAA Training Certification. Failure to provide the Minimum Qualifications could result in disqualification of a bid.

Question #6 Will you be providing dry ice (if dry ice is needed)?
Answer #6 No, DSH will not be providing dry ice and does not anticipate requiring dry ice for transportation. However, DSH will provide ice packs when necessary.
Question #7 Question regarding HIIPA and Blood born pathogen training. Currently, we have a packet for HIIPA and BBP training that our drivers sign off on. Will their signature page serve as a certificate? One of the bid stipulations is to include a certificate for BBP and HIIPA training for all drivers we plan to use to service
ASH.
Answer #7 Yes, a signature page from an organized training is acceptable to DSH to act in place of a certificate.
All other requirements and conditions of this IFB remain in full force and effect. If you have any questions or need assistance, please contact me via email at ASHProcurement@dsh.ca.gov.
