SUPPLEMENTAL INFORMATION LETTER NO. 1
April 27, 2023
BID NO. 234039
FOR: MEDICAL SUPPLIES
CLOSING DATE: MAY 17, 2023 AT 11:00 AM (LOCAL TIME)
The following is questions and answers received on this procurement:
Q: Will this bid be awarded to only one vendor or more than one? Who is the current vendor?
A: Only one vendor will be awarded based off low bid. Bound Tree Medical is the current vendor.
Q: On the Proposal page Attachment 1 there are 2 tabs, appears items on tab 1 are on tab 2 as well, are we pricing both tabs?
A: Just the tab sheet titled “Bid List” needs to be filled out.
Q: If there is one or more manufacturer listed on a line- and Brand indicates NO PREFERENCE-do we need to submit a SUBSTITUTION REQUEST FORM if bidding something not listed? Or can we bid a substitution and make a note in the comments of the manufacturer being bid?
A: For items listed “No Preference”, a substitution form does not need to be filled out, you can write the brand in on the comment section.
Q: When requesting to bid a sub, is it sufficient to submit literature before 5/10, or are samples required as well?
A: For substitutions, just fill out the substitution form and any literature you’d like to include by May 10, 2023 @ 5:00 PM (Local Time). If the Contract Administrator needs any samples, he can let us know at that time.
Q: ESCALATION/DE-ESCALATION AGREEMENT- Manufacturers will not provide documentation of price increases that we can share with customers, and we cannot share invoice or payment information from our manufacturers. Will you accept a price increase at the time of renewal without this documentation from a manufacturer?
A: For the price increases, it would be up to the department to accept it, however it can’t be more than 15%
Q: We are still referring to the 1st tab BID LIST for substitutions,- - we do not need to send any information regarding substitutions we may offer for the 2nd list?
A: Correct
Q: On the 1st list lines 175-177 we still need the manufacturer part # in order to bid correctly, the description isn’t enough to know what the bid is asking for.
A:
Q: Pharmaceutical Section column UNIT- are you asking for a price on a box/carton/pack or EA
UOM in order to supply a TOTAL PRICE we need to know what the UOM is
Example line 185 Adenosine Vial qty is 20- is this 20 EA or 20 paks?
A: UOM = EACH
Q: Glove section I understand we can bid any glove- however they are either 500/case or
1000/case- so it will make a difference in the total price when comparing bids from each supplier- do you have a preference on the qty. per case?
A: 1000/Case lines 175-177 we still need the manufacturer part #
