No. Each RFP proposal can determine the format to match the budget items being included.

2. Is there an example proposal you all can send to me so I know and understand exactly what to submit?

The State does not provide example proposals as each RFP submission is different and designed by each entity and the information being presented

3. Should my proposal's budget reflect this travel necessity or am I ineligible due to the distance?

Budget line items should include travel expenses for in-person meetings, trainings, or presentations.
