About this file
This document is an Application Response Form for a Request for Response (RFR) issued by the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health (DPH) for Emotional Health Programs for Massachusetts Nursing Home Residents, RFR #225827. The RFR seeks proposals for programs that will improve quality of life and strengthen health outcomes for nursing home residents, specifically addressing the requirements of the CMS CMP programs. Bidders must complete and submit the Application Response Form, along with various attachments such as an organizational chart, CVs of key personnel, a Supplier Diversity Program (SDP) Plan Form, a Proposed Subcontractor List, a Program Budget, and a Scope of Work. The deadline for submitting proposals is not specified.
The RFR requires bidders to be active in the Massachusetts Management Accounting and Reporting System (MMARS) and to submit vendor activation documents if necessary. Bidders must also propose one or more SDP Partners to meet their SDP Commitment, and certified diverse bidders cannot list their own companies or subsidiaries as SDP Partners. The Program Budget and Budget Narrative must demonstrate how each budget item supports the overall program. Subcontracts are subject to DPH approval, and the Proposed Subcontractor List must be completed if subcontractors are used.
