STATEMENT OF WORK (SOW)

FY16 GYM SERVICE MAINTENANCE AGREEMENT

FOR CG INSTITUTE

1. SCOPE

As required by the United States Coast Guard (USCG), COMDTINST M6200.1B, requires “All active duty members shall engage in fitness activity as outlined in their Personal Fitness Plan, Form CG-6049, a minimum of 180 minutes per week. It is strongly recommended that the fitness plan include 150 minutes of cardio respiratory activity and 30 minutes of strength training.” The gym needed to fulfill the physical needs of the USCG and CG Institute must be open and available during normal business hours, be in close proximity to the CG Institute, offer a variety of cardiovascular equipment as well as various strength conditioning/ resistance training equipment.

2. BACKGROUND

The CG Institute and CG CITAT units will both be using the memberships for 25-30 members. In order to have an operationally physically ready workforce and to promote general health and wellness, it is imperative to have a gym membership for both units that is a reasonable distance from the units in order to maximize health benefits while minimizing travel time that takes away from the time spent at work..

3. TECHNICAL REQUIREMENTS

3.1 Membership Type and Volume: The Contractor shall provide USCG Institute and USCG CITAT 25-30 memberships with individual sign-in numbers. Members shall log in to the gym upon entry every time. Members may use the gym multiple times per day.

3.2 General Requirements: The Contractor’s fitness staff shall be professional and knowledgeable in general fitness programs and fitness plan development as well as have in depth knowledge on the safe use of the equipment within the facility. The Contractor shall maintain clean cardiovascular machines and free weights, showering facilities, and separate men/women locker rooms.

3.3 Location: The gym is within half a mile distance from both the CG Institute and CG CITAT. Due to time constraints, the gym needs to be within 5 miles and/or a 10 minute drive from the unit.
