1. Contracting Activity: Department of Veterans Affairs, Network Contracting Officer (NCO 15), intends to establish a Firm Fixed Price one time purchase Contract for the following medical center:

Station # VAMC Name City State

657 2401 W Main St.

Marion Illinois

Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This is a Firm fixed price, brand name, Sole source delivery order For the Marion VAMC. current device is the evaccusled and is only used for inpatient beds but is no longer compatible with ICU beds or bariatric beds. This product is not available to clinic areas as it needs a mattress to be used with the device. This acquisition is conducted under the authority FAR 13.106-1 with the level of quality required because the supplies or services are unique or highly specialized. This unique or highly specialized product is the med sled. This is a no lift device that will transfer the person from a hospital bed, chair, or floor keeping them flat, secure and safe. It will allow them to be transferred down the stairwell with out manual lifting. This will reduce the risk of injury to staff members performing the task and keeps the patient safe while providing a safe and efficient way of evacuation. Currently both Marion campus and Evansville campus do not have a sufficient evacuation devices.

2. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Size of storage for the amount needed to cover both the Marion and Evansville campus. It has a storage case that can be mounted on the walls and out of the way of patient flow in the hallways. The material holds its shape while transferring a patient down the stairs. The clasp that is secure to the handrail is larger and the strap does not get stuck or tangles during the evacuation.

3. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted. Our current device is the evaccusled and is only used for inpatient beds but is no longer compatible with ICU beds or bariatric beds. This product is not available to clinic areas as it needs a mattress to be used with the device. Three products that seem to meet the needs of the facility was brought in for a trial. Staff was shown a demonstration and returned demonstration with each product. The local fire department for Evansville and Marion were also invited and attended these sessions. The Evansville demonstration was conducted on 09/05/2019 and for Marion the demonstration was conducted 09/06/2019 for all three products. Storage was an issue with sled to go. It is big and bulky. There was also no strap to secure the patient to the stair rail.

The staff member would have to stand in front of the patient being evacuated and be the braking https://www.acquisition.gov/?q=/browse/far/13

The sled did not hold its shape even with a person in it. The straps that secured the patient would loosen a bit while going down the stairs. It was also noted that the strap on the supersled would get tangled in the clasp that was secured to the handrail. In the event of an emergency evacuation, this could pose a risk to the staff member and patient. This trial and market research was conducted due to an event that occurred requiring evacuation and a staff member was injured.

4. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.

Note: COs are required to make a determination of price reasonableness IAW FAR 13.106-3. See the
