About this file

This is a Qualification Form attachment issued by the State of Minnesota for a solicitation related to herbicide application services, specifically for broadcast forest herbicide application or roadside and rights-of-way application using ground equipment. The solicitation requires contractors to respond with completed qualification forms demonstrating compliance with specified experience and licensing requirements. The form must be submitted with all bid responses, and responses received without the current version of the qualification form will be rejected as non-responsive. The State reserves the right to clarify and verify all submitted qualification information before making an award determination, and contractors failing to meet the stated qualifications will have their responses rejected.

The qualification requirements specify that the primary contractor must have a minimum of two years of experience in broadcast forest herbicide application or roadside and rights-of-way application using ground equipment, while all on-site applicators must possess a minimum of one year of herbicide application experience with ground equipment. Additionally, the contractor and all on-site applicators must demonstrate the ability to identify target invasive plants including spotted knapweed, common tansy, wild parsnip, non-native thistle, and reed canary grass in their early summer growth stage. All applicants must hold a current license recognized by the Minnesota Department of Agriculture to commercially apply herbicides, specifically MN Category A plus G or J or an equivalent license, with proof of current licensure required at the time of bid submission. The form requires applicants to provide applicator names, positions, years of experience, types of work performed, applicable MN license categories, and license numbers, along with certification that all information provided is complete and accurate.
