SECTION C – STATEMENT OF WORK
Title: NFL Alumni (NFLA) Health Shingles Vaccine Community Outreach and Education Project
SECTION 1 – BACKGROUND
About 1 out of every 3 people in the United States will develop shingles, and an estimated 1 million people get shingles each year in this country. Shingles disease can cause acute pain and suffering with relatively high ‘risk perception’ of the disease—meaning people who have heard of this disease do take it seriously. Despite this, shingles vaccine coverage among adults aged ≥50 and ≥60 years in 2020 was 29.4% and 39.1%, respectively (in 2020). Of note, White adults had higher coverage compared with African American, Hispanic, and other adults.
While shingles vaccination among adults aged 60 and over increased from 6.7% in 2008 to 34.5% in 2018, non-Hispanic white adults (38.6%) were approximately twice as likely as non-Hispanic black (18.8%) and Hispanic (19.5%) adults to have ever received a shingles vaccine. Vaccination coverage was highest for those who were not poor and those who had more than a high school education. In addition, the percentage of adults aged 60 and over who ever had a shingles vaccination ranged from 26.3% in the Southeast region to up to 42.8% in the West North Central region of the United States.
Building awareness of shingles as a disease and creating confidence in the important role of shingles vaccination at a community level is essential. Because many factors influence vaccine decision-making, including contextual factors (e.g. cultural, social, political), individual and group factors, and vaccine-specific factors, the CDC relies upon trusted community figures to disseminate health information.
The NFL Alumni Association (NFLA) is an independent, non-profit organization founded in 1967 by the oldest and most recognizable national organization of retired professional athletes. More than 10,000 of these former NFL players are connected to the NFLA. The NFLA focus is both national and local, with more than 40 chapters across the US. The NFLA has a history of successfully working with CDC and other public health organizations, including supporting current CDC COVID-19 vaccination efforts.
Through its work with CDC, NFLA has engaged more than 100 retired and active NFL players to serve as Ambassadors to promote vaccination in 20 markets, through 59 PSAs, 21 million social media impressions, more than 450 media stories, and 24 events, partnering with 19 state and local health departments. Building on its work with CDC, NFLA is currently working in partnership with the Tennessee Department of Health, on community-based COVID-19 vaccine education and outreach, engaging more than 20 retired NFL players who serve as Ambassadors, who are joining vaccination events and using campaign messages to promote vaccination uptake, making this the ideal time to expand upon all of these public health efforts to include shingles vaccine awareness.
SECTION 2 – PURPOSE/OBJECTIVE
The overall purpose of this contract is to utilize the NFLA Alumni organization and Ambassador network in the Mid-Atlantic and South Atlantic regions, where several states are experiencing lower than average shingles vaccination rates, to help educate adults aged 50+, especially in African American communities, about shingles disease and actively promote the benefits of vaccination.
CDC formative research has shown that trusted individuals can vary widely, and may include healthcare workers or, occasionally, media figures or celebrities. NFL Alumni are trusted in their community as beloved representatives who are also seen as more accessible celebrities. Their testimonial experiences and frank conversations about vaccination will build trust with the process and help direct their targeted community toward valid sources of information, and ultimately, to seek out shingles vaccination.
The NFL alumni are in many cases revered and respected household names for this target audience, including world champions, Pro Bowlers, and Pro Football Hall-of-Famers. In addition, nearly 75% of their organization features African American and Latino players who are influential not only in their communities, but across the country, and are in the targeted age range themselves for increased risk of shingles and eligibility for shingles vaccine.
SECTION 3 – SCOPE OF WORK
The contractor shall facilitate collaboration among NFL Alumni and CDC, state, and local public health officials, as well as other local leaders, to create shingles vaccine awareness digital and social media assets (such as social media posts, tweets, and other media as possible, including radio or online video public service announcements), promoting through NFLA and NFLA-related partners’ social media channels, as well as providing them for placement on the CDC and state and local health department social media outlets as available.
The contractor shall help organize , promote with traditional or social media, and provide players as ambassadors to up to 2 local events (area determined by need and capacity) within states in the Mid-Atlantic or South Atlantic that have lower than average vaccination rates, either in-person if safety protocols allow, or alternative virtual ‘town halls’ or other options if public health measures restrict in-person events. The coordination of these events will engage public health partners such as the local or state health departments and/ or community partners to create a shingles vaccination awareness event.
SECTION 4 – TASKS TO BE PERFORMED
Task 1) Identify potential NFLA ambassadors, especially those who may have had shingles disease or shingles vaccination. Recruit and rapidly engage, train, and support these ambassadors to be ready to educate others and speak about shingles vaccination in their social media, as well as preparing them for outreach in the the Mid-Atlantic and South Atlantic areas, with an emphasis on states that are experiencing lower than average vaccination rates and at-risk African American communities.
Task 2) The contractor shall create personal and compelling testimonies as digital and social media assets with ambassador athletes for digital and social media outlets, including using the players’ existing outlets to promote these assets.
Task 3) The contractor shall share these assets with all of NFLA’s existing national network of more than 40 chapters, as well as with CDC to use on CDC’s and CDC partners’ existing social media channels as well.
Task 4) The contractor will coordinate with with local and/or state-based public health agencies and clinical experts for up to two in-person health education and outreach events (or virtual alternatives are acceptable if public health measures restrict in-person events). They will engage as appropriate with other community partners( for example, the local or state health departments, community leaders, pastors and other religious leaders, local clinician community, local celebrities, local media, etc.) to help promote an outreach and education event in the community . The locations (if not virtual) shall be determined with CDC staff based upon vaccination rates and demographics of the community, and feasibility of travel.
Task 5) The contractor shall provide a report with the metrics and evaluation of all activities, as well as the impressions and engagements from all digital or social media placements in a report to the COR.
SECTION 5 – GOVERNMENT FURNISHED MATERIALS
CDC shall provide all public health resource content and approved messaging around shingles disease, shingles vaccination, and other scientific or public health recommendations.
SECTION 6 – PLACE OF PERFORMANCE AND CONTRACT SUPPORT HOURS
SUBSECTION A – PLACE OF PERFORMANCE
The contractor’s place of performance will be off-site.
SUBSECTION B – CONTRACT SUPPORT HOURS
The contractor’s support hours are estimated to be 20 hours per week Monday-Friday.
Section 7. (Optional) Travel The total cost for travel should not exceed $15,000 for planning purposes.
The contractor shall comply with all terms and conditions of set forth in the Federal Travel Regulations and CDC policy and guidelines.
The educational event(s) may occur at sites identified and approved by CDC as 1) beneficial for the at-risk target audience and 2) focused areas of low shingles vaccination rates in the Mid-Atlantic or South Atlantic states or other states as identified with CDC that have low vaccination status.
Persons traveling to these events may include the project manager and other staff, as well as select NFLA ambassadors to support the project event(s) on-site site.
Travel does not include conferences. Conference Approval Management Systems (CAMS) approval is required in advance of any travel for conferences. If the need for conference travel occurs, the contract will be modified to separately add funding.

Section 508 also requires that individuals with disabilities, who are members of the public seeking information or services from a Federal agency, have access to and use of information and data that is comparable to that provided to the public who are not individuals with disabilities, unless an undue burden would be imposed on the agency.
