Each subfactor under technical will receive an individual rating of “acceptable” or “unacceptable.” An offeror’s technical proposal must receive an “acceptable” rating on every subfactor for the overall technical proposal factor to receive an “acceptable” technical rating. An offeror’s proposal that receives an “unacceptable” on any one subfactor will receive an overall “unacceptable” technical proposal rating.

The Offeror shall provide a technical approach to perform tasks required to operate the activated sludge wastewater plants at Shaw AFB, SC (Sumter County) and Wateree Recreation Facility (Kershaw County) in accordance with 1) PWS, 2) SC Department of Health and Environmental Control (DHEC) rules and regulations governing wastewater treatment plants, 3) South Carolina issued National Pollutant Discharge Elimination System permits (currently # SC0024970 and SCG570034).
B. The Offeror shall provide a technical approach to be used to haul and dispose of (off-site) large volumes of wastewater sludge generated at Shaw AFB (approx. 850,00 gallons/year) and at Wateree Recreation Facilities (approx. 54,000 gallons/year)
C. The Offeror shall provide their technical approach to be used to perform the laboratory analysis requirements of the PWS, SC DHEC rules and regulations, and South Carolina issued National Pollutant Discharge Elimination System permits (currently # SC0024970 and SCG570034) for Shaw AFB and Wateree Recreation Wastewater Treatment Plant Facilities.
Subfactor 2: Management Approach:

Offeror shall provide an approach to perform preventative and emergency Wastewater Treatment Plant equipment at Shaw AFB and Wateree to ensure the plant(s) are operating in accordance with standard operating parameters, meeting the requirements set forth in the NPDES permit(s).
2. PART II – PRESENT/PAST PERFORMANCE INFORMATION.
*Only references for recent and relevant present/past efforts/contracts are desired.
A. Quality and Satisfaction Rating for at least two but not more than four contracts performed in the last 5 years. These contracts shall be similar and scope and complexity as the contract in this solicitation. Provide any information currently available (letters, metrics, customer surveys, independent surveys, references, etc.) which demonstrate customer satisfaction with overall job performance and quality for relevant past and present efforts or contracts that are similar in scope, magnitude of effort and complexities this solicitation requires. Explain any/all corrective actions taken in the past, for substandard performance and any current performance problems such as cost overruns, extended performance periods, excessive warranty calls, etc.
B. Performance Surveys. The government will evaluate the quality and extent of offeror’s performance deemed relevant to the requirements as specified in Shaw Air Force Base and Wateree Recreation Area requirement (PWS, RFP). The government will use information submitted by the offeror and any other sources of information available to the government to assess past performance. Provide a list of at least two but no more than five (5) of the most recent and relevant contracts performed for Federal agencies and commercial customers within the last five (5) years. Recent is defined as contract completed or ongoing within five (5) years from the release date of this solicitation.
Relevant performance is defined as operating, maintaining, and repairing a Wastewater Treatment System that is similar in scope, magnitude of effort and complexities as defined in the Shaw Air Force Base and Wateree Recreation Area Performance Work Statement. Aspects of relevancy include, but not limited to: system design, gallons of water per day, operator license type, 24/7 operations; and simultaneously operate, monitor, maintain and repair all facilities as specified in the Operation and Maintenance of Wastewater Treatment Facilities—Shaw Air Force Base and Wateree Recreation Area, South Carolina.
Furnish the following information for each contract listed for Present/Past Performance:
1. Company/Division Name
2. Product/Service
3. Contracting Agency/Customer
4. Contract Number
5. Contract Dollar Value
6. Period of Performance
7. Verified, up-to-date name, address, & telephone number of the contracting officer
8. Comments regarding compliance with contract terms and conditions
9. Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.
C. Joint Venture Agreements. Joint venture agreements must be received by the Small Business Administration (SBA) prior to proposal due date and approved before award of any resultant contract. If you are contemplating a joint venture on this project, you must advise your assigned SBA business opportunity specialist (BOS) as soon as possible. It is also recommended that the agreement be submitted as soon as practicable to ensure compliance with established regulations. Any corrections and/or changed needed can be made only when your BOS has adequate time for a thorough review before the proposal due date. No corrections and/or changes are allowed after time of submission of the proposal.
3. PART III – PRICE PROPOSAL
The Contracting Officer has determined there is a high probability of reasonable price competition for this acquisition.
A. Complete Blocks 12, 17 and 30 of the SF1449, as well as signing or acknowledging any amendments. In doing so, the offeror accedes to the contract terms and conditions as written in the request for proposal.

C. Complete the necessary fill-ins and certifications in provisions. The provision FAR
52.212-3, Offeror Representations and Certifications – Commercial Items, shall be returned along with the proposal.
D. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. The total evaluated price (TEP) will consist of the sum of all Contract Line Items for base year, option years, and 6-month extension of services IAW FAR Clause 52.217-8 Option to Extend Services.
Offerors shall include a price for the Option to Extend Services in the pricing proposal submission.
Note: Evaluation of options shall not obligate the Government to exercise any, or all options. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not negotiations are held after its receipt, unless a written notice of withdrawal is received before award. The Government intents to award a contract without interchanges with respective offerors. The Government, however, reserves the right to conduct interchanges, if deemed in its best interest.
