1. This is a firm-fixed purchase order. It includes two option periods of performance. (d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

March 17, 2016 Background
The mission of the Division of Scientific Education and Professional Development (DSEPD) is to provide leadership in public health training and education, and manage innovative, evidence-based programs to prepare the health workforce to meet public health challenges of the 21st century. DSEPD’s activities fall into 5 main areas:
Recruit New Talent Into Public Health - DSEPD programs recruit, select, and provide in-depth, on-the-job, experiential learning for future public health professionals. The DSEPD-managed programs include: Epidemic Intelligence Service (EIS), Laboratory Leadership Service (LLS), Preventive Medicine Residency/Fellowship (PMR/F), Steven M. Teutsch Prevention Effectiveness (PEF) Fellowship, Public Health Informatics Program (PHIFP), and Presidential Management Fellows (PMF) program. DSEPD also manages two student programs: Epidemiology Elective for Senior Medical and Veterinary Students and CDC Hubert Global Health Fellowship. CDC’s Career Paths to Public Health program has a portfolio of science, public health, and epidemiology educational programs, resources, and activities for middle and high school students and for middle and high school science, technology, engineering, and mathematics (STEM) educators to introduce public health into the classroom and as a career choice.
Provide Training for the Existing Workforce - CDC’s training programs aid all health professionals. In November, 2010, CDC launched the CDC Learning Connection (www.cdc.gov/learning), a one-stop web resource for public health learning products and training available from CDC and partner organizations, for all health professionals. In August 2011, CDC expanded the CDC Learning Connection to include CDC TRAIN, a searchable learning management system in collaboration with the Public Health Foundation, so that now thousands of courses are immediately available to the public health community.
Academic Partnerships – DSEPD manages CDC’s cooperative agreements with academic partners. The new cooperative agreements, which started in FY’ 12, focus on improving health outcomes, making education more practical, and aligning experiential opportunities for public health, medical, and nursing students with local public health priorities. The grantees include the Association of Schools of Public Health (ASPH), Association for Prevention Teaching and Research (APTR), Association of American Medical Colleges (AAMC), and the American Association of Colleges of Nursing (AACN).
Service and Technical Assistance - DSEPD provides epidemiologic, informatics, and economic/quantitative policy analysis technical assistance to CDC, global, state, tribal, local, and territorial partners through Epi-Aids, Info-Aids, and Econ-Aids. These are initiated and completed at the request of the public health agency or partner. DSEPD also uses its expertise and experience in designing program curricula and managing programs to help colleagues across CDC design and manage their own fellowship programs.
Workforce Strategy - DSEPD provides cross-cutting leadership of CDC workforce activities. DSEPD works with other federal agencies, academic partners, and non-governmental organizations to strengthen the public health workforce by influencing policy and improving the quality, availability, and accessibility of training and workforce development activities. DSEPD addressed strategic workforce issues by collaborating with HRSA to advance the public health workforce enumeration project, with the Department of Labor (DOL), to establish PHIFP as a DOL Registered Apprenticeship; and with the Office of Public Health Preparedness and Emergency Response (OPHPR) and the Public Health Surveillance and Informatics Program Office (PHSIPO) to create the Career Informatics Field Officer (CInFO) program.
DSEPD identified a need to implement an overarching, comprehensive assessment and evaluation framework to be used by all DSEPD fellowships for accurately assessing and reporting the competence of its graduates, for evaluating the success of the fellowships in selecting and developing competent fellows, and for identifying areas for improvement to competencies, curricula, or processes. DSEPD researched viable assessment methods and identified the electronic portfolio of evidence as the single most effective and reliable method of assessing competence of fellows in an applied, on-the-job learning environment. In addition, fellowships have been using a wide variety of methods for portfolio management, and DSEPD would like to have a common technology solution for all the fellowship programs.
DSEPD defined its functional, technical, and educational requirements and conducted a study to explore the use of commercial, off-the-shelf (COTS) electronic portfolio products as an alternative to developing a secure portfolio management system that meets the needs of its fellowships. Motivations for using COTS include reduction of overall system design and development costs and reduced maintenance costs. The results of the study indicated that a COTS product could meet DSEPD’s rigorous set of requirements.
A COTS electronic portfolio of learning evidence product will be integrated within the current CDC Fellowship Management System (FMS) to provide authentic assessment of applied, on-the-job performance requirements based on required fellowship competencies. The tool will include reflective and content components. Detailed instructions for fellows, supervisors, and other assessors will be included for completion, submission, and assessment of work products and other evidence of work performed and learning that occurred in an authentic performance environment. The tool will also include rubrics for scoring, validated by subject matter experts, to support assessors, enhance their confidence in their ability to provide valid and reliable feedback and assessments, and to ensure reliability of assessment across assessors. Moreover, the tool will include a secure survey system that can be used for cognitive tests, such as pre- and posttests, as well as for evaluating fellowships and fellowship activities; for example, in conducting supervisor and fellow midyear and exit surveys. Survey data will also provide evidence for making changes to curricula or other fellowship components or processes.
Scope of Work
Under this contract, the contractor shall provide a COTS electronic portfolio of evidence product including technical services, training, and support for the Division of Scientific Education and Professional Development (DSEPD) to further refine the overarching evaluation framework with a crosscutting system for tracking, providing feedback, and assessing fellows’ attainment of competencies. The contractor will perform the following activities and tasks based on the priorities established by DSEPD and funding availability:
1. Provide a COTS product that meets all DSEPD technical, functional, and educational requirements
2. Provide ongoing technical and educational support for the current COTS product behind the CDC firewall
3. Support DSEPD in the integration of the COTS system with the existing Fellowship Management System if the need is identified
4. Support DSEPD in the transition from the existing version behind the CDC firewall to an external-facing (web-based) interface if the need is identified
· Provide technical and educational support for the rollout to production
· Provide input for security documentation requirements as a 3rd Party Site
5. Provide technical and educational support for the rollout to production
6. Provide ongoing technical and educational support for the length of the contract
7. Provide annual system updates in accordance with critical dates and milestones set by the government

The vendor is responsible for documenting controls to level deemed acceptable by the DSEPD Information System Security Officer (ISSO) and the CDC Office of the Chief Information Security Officer (OCISO).
26. The vendor will provide application support required to address valid security deficiencies identified with the acceptable tool required by the CDC policies and standards for Web application security – currently AppScan Enterprise Edition. The vendor will provide detailed documentation to support any assertion that findings from an application scan are invalid or false-positive findings.
27. Vendor will provide support necessary to comply with the Federal Information Security Management Act of 2002 (FISMA) as applicable, including support of the system authorization process that results in the an authorization to operate (ATO) issued by the CDC Office of the Chief Information Security Officer
Task 2 -- Support CDC and DSEPD in the integration of the COTS system with the existing Fellowship Management System
· Contractor will provide integration support and ongoing technical support for the length of the contract
Task 3 -- Provide educational support when necessary related to use of the tool

Disposition and Use of Data: All materials developed or information of whatever nature resulting from work being performed under this contract shall not be submitted for publication or dissemination without the prior review and written approval of the Project Officer and Technical Monitor.
Location of Work: Both offsite and onsite, dependent on the task and the requirements of the project. Training and other work conducted onsite will be performed at 2400 Century Center, Atlanta, GA 30345 or other designated local sites. A workstation with computer and printer will be made available to the contractor when working onsite.
Needle Exchange
No funds appropriated in the FY 12 Appropriations Act and obligated to this contract may be used to carry out any program of distributing sterile needles or syringes for the hypodermic injection of any illegal drug.

Respondents to this solicitation must also provide any additional detailed information necessary for determining applicable Section 508 standards conformance, as well as for documenting EIT products and/or services that are incidental to the project, which would constitute an exception to Section 508 requirements. FAR 52.217-9 Option to Extend the Term of the Contract (Mar 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 10 days; provided, that the Government gives the Contractor a preliminary written notice of its intent to extend at least 10 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
