This is a Contract Amendment No. 1 to Contract No. 15-91579-601 between the State of Wisconsin Department of Administration and AT&T Global Services for AT&T Combined Voice Network Services for Centrex, Long Distance and Voice Processing. The amendment extends the contract for a five-year period from July 1, 2011 through June 30, 2016, covering the Combined Voice Network Services Agreement, Addendum 2 (Integration Information Network Service), Addendum 4 (Custom Billing and Collection Service), and Addendum 6 (Standard Terms and Conditions). The contract allows authorized users including Wisconsin municipalities, counties, cities, villages, towns, school districts, and federally recognized Indian tribes to participate under cooperative purchasing provisions.

The amendment establishes new tiered pricing for Centrex services based on the total number of lines in service, ranging from $9.25 per line per month for 70,000+ lines to $14.75 per line per month for accounts with fewer than 20,000 lines. Local services receive a 20% discount off prevailing tariff prices, with service order charges of $44.00 and line connection charges of $20.65. The Caller ID with Name feature is reduced from $1.50 to $0.25 per unit per month with no installation charge. The contract includes provisions for migrating Centrex lines to qualifying services from AT&T affiliates, with various line types contributing to the total count (Centrex lines = 1, ISDN Prime = 23, etc.). AT&T must provide dedicated staff support including one Service Manager, one Account Manager, one Sales Support Manager, six Service Representatives, and seven Maintenance Personnel in Madison, Wisconsin.
