About this file

This is Amendment #2 to Contract 15-91579-601, Addendum 5 for Hosted IP Service between the State of Wisconsin Department of Administration and AT&T Global Services on behalf of AT&T Corporation. The amendment was executed in March 2009 and modifies the existing contract for AT&T Combined Voice Network Services for Centrex, Long Distance and Voice Processing by adding new service definitions and pricing schedules for Local Voice Over IP (L-VIP) services.

The amendment introduces L-VIPS (Business Local Voice Over IP Service), a managed VoIP solution where AT&T manages Customer Equipment including Cisco Call Managers and Gateway Routers leased to the Customer. Key services include equipment leasing with installation and management charged on a per-station basis at $11.95 monthly recurring rate with a $75.00 non-recurring charge. Gateway Router leasing ranges from $3,800 to $9,100 for various configurations supporting 2-8 Centrex lines or 1-4 ISDN Primes. Additional features are available including LMR Connect ($5.00/user monthly), Rapid Broadcast ($4.75/user monthly), Call Center Record ($5.00/user monthly), and Unity Voice Mail ($5.50/user monthly), along with various one-time charges. The contract includes centralized trunking capabilities, emergency 9-1-1 services, and integration with the Customer's voice processing platform, with all pricing reflecting a 0% discount structure.
