JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1

(OVER MICRO-PURCHASE THRESHOLD($3K) BUT NOT EXCEEDING THE SAT ($150K))

IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). Note: Statements such as "only known source" or "only source which can meet the required delivery date" are inadequate to support a sole source purchase.

1. PURCHASE REQUEST OR REQUISITION NUMBER:

1A. PROJECT/TASK NUMBER

None

1B. ESTIMATED AMOUNT:

$40,000

2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE:

The St. Louis VA Health Care System is seeking a virtual health and wellness service that empowers older adults experiencing difficultly in adopting current technology, has limited or declining mobility, and/or suffers from social isolation/depression. The virtual health and wellness service provider will deliver, install, maintain and remove (when use is no longer needed) all leased hardware including a broadcast kit, HIPPA compliant Android streamer, HD web camera, and customized remote control with embedded microphone. The virtual health and wellness service will allow older Veterans to engage in a socially and physically active lifestyle from the comfort of their home. Internet service is a requirement for use of the virtual health and wellness service. If the user cannot maintain or gain access to internet services, cellular internet will be provided to the user via the virtual health and wellness company. The virtual health and wellness service will provide St. Louis VAMC with pre-recorded programming/content, as well as the ability to broadcast audio/video in real-time to multiple users. The virtual health and wellness service will allow the St. Louis VAMC to build a customized virtual library for viewing targeted patient education, emotional well-being programming (social isolation and loneliness) and other virtual content to promote preventative care and increase overall wellbeing of Veterans. Vendor supported programming focuses on social (empowerment lectures, interactive bingo and trivia, music and culture listening), physical (health education, core exercise, seated yoga, and Qigong) and emotional (guided imagery, meditation, laughing yoga, and coping education) wellbeing.

3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED:

The Uniper solution is proprietary to Uniper Care Technologies Inc., which is the only provider in the market that offers an interactive & personalized tech enabled service for older adults through their standard television set. The Uniper program and service aims to improve the overall health and well-being of the elderly population suffering from 1. anxiety, 2. depression, 3. isolation, 4. loneliness, and 5. reduced mobility. No other service vendor was found to target these 5 risk factors that impact Veteran suicide.

Uniper’s solution consists of several proprietary components, that are unique to the Company:

1. Accessible- HIPAA compliant telecare platform tailored for older adults to increase engagement and compliance. The platform includes both proprietary hardware and software and was proven to be adopted by older adults including the ones that are not comfortable with technology.

2. Analytical rule-based software engine- Used for building a risk profile to drive a personalized and tailored service/interventions to the Veterans home television.

3. Evidence based - The virtual community health and wellness programming service (virtual programming) for older adults is backed by pre/post studies. The service is aimed to promote preventative care and healthier lifestyle with a focus on – social isolation and loneliness, fall risk reduction, depression and anxiety.

4. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: Uniper Care Technologies Inc. is the only vendor in the market providing a virtual whole health solution with minimal adoption to current technology. The personalized tech-enabled service for older adults is aimed to promote preventative care (emotional, social and physical focused outcome) and build social communities via virtual interventions. Uniper Care Technologies Inc uses the TV as the main medium to conduct these live and interactive interventions. Uniper Care Technologies Inc. is the only vendor in the market that supports a mobile app allowing family members to text, share video/photos with the Veteran through the television.

5. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED:

Market research was performed (searching the internet, discussions with other healthcare agencies etc.) and no other service vendor was found that offers the same services and technology. Uniper Care Technologies Inc. is the sole provider of an innovative unique solution.

1. A search of “Telehealth” + “Anxiety” + “Depression” + “Isolation” + “Loneliness” + “Mobility” produced no viable competitors from Google search engine.

2. A search for “Telehealth using the TV” produced 1 telehealth platform. The platform found from the search is tailored to the healthcare provider, not the patient. The telehealth platform found does not target emotional well-being programming.

3. A search of “Virtual health” + “Telehealth” + “Anxiety” + “Depression” + “Isolation” + “Loneliness” produced no viable competitors from GSA e-Library.

4. A search for “Anxiety” + “Depression” + “Telehealth” + “Service” using Google search produced 2 vendors. MDLIVE does not adapt to the patient TV, nor does it offer scheduled daily programming. MDLIVE offers face to face access from therapists. The second vendor is called VSee with an array of telehealth solutions but does not adapt to the patient TV and lacks a database for uploading VA produced education.

5. A search for “Anxiety” + “Depression” + “Telehealth” + “Service” using Yahoo search engine produced no viable candidates for comparison.

6. A search for “Anxiety” + “Depression” + “Telehealth” + “Vendor” using Yahoo search engine produced 2 vendors. Brightside is a telehealth service providing personalized cognitive therapy for treatment of anxiety and depression but does not have adaptability to TV and is a boutique vendor with a solution scaled for an office setting. The second telehealth vendor is Clocktree and they do not support use from the patients TV, and requires adoption to modern technology.

7. A search for “Loneliness” + “Isolation” + “telehealth” using Bing search engine produced one telehealth vendor. Advanced MD is a boutique vendor that specializes in private practice telehealth. This technology does not provide integration to the patient’s TV nor does it have daily scheduled programming to improve lifestyle.

5. CONTRACTING OFFICER’S CERTIFICATION: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
