About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute 45A.395 for bidders and offerors responding to state and local contract opportunities in Kentucky. The document serves as a prerequisite certification for determining bidder responsibility and requires the undersigned to attest under penalty of perjury that neither they nor anyone acting on behalf of the bidder has knowingly violated Kentucky's campaign finance laws, and that contract award will not result in violations of such laws. The statement must be completed with the bidder's company name, signatory information, title, and date, and submitted as part of the proposal package for any applicable Kentucky procurement opportunity.
The form defines "knowingly" as awareness or should-have-been awareness that conduct violates applicable statutes. This certification is a mandatory compliance document and does not contain pricing information, specific project details, deliverables, timelines, or award criteria. Rather, it functions as a legal prerequisite affirming the bidder's eligibility and good standing regarding campaign finance compliance under Kentucky law. Failure to submit this sworn statement or providing false information would render a bidder non-responsive or ineligible for contract consideration.
