About this file
This is a sworn statement form required by Kentucky Revised Statutes Section 45A.395 for the City of Owensboro's procurement of stormwater culvert inspection services. The statement serves as a prerequisite for determining whether a bidder qualifies as a responsible offeror and must be completed and submitted by all prospective contractors. The form requires the bidder's authorized representative to swear under penalty of perjury that neither they nor anyone acting on behalf of the bidding entity has knowingly violated any provision of Kentucky's campaign finance laws and that awarding the contract will not result in violations of such laws. The document defines "knowingly" as awareness or reasonable awareness that conduct or circumstances are of the nature described by statute.
The statement must be signed, dated, and notarized before submission as part of the bidding package. Completion of this form is mandatory for all bidders seeking to compete for the City of Owensboro stormwater culvert inspection contract. No exceptions or waivers to this requirement are permitted, as it represents a fundamental responsibility determination under Kentucky procurement law.
