a. The Contractor starting construction stage(s) of the contract.
b. Processing Contractor's invoice(s) for construction activities/items of work.
c. Review of any schedule updates.
Submittal of the Baseline Construction Schedule, and subsequent schedule updates, is understood to be the Contractor's certification that the submitted schedule meets the requirements of the Contract Documents, represents the Contractor's plan on how the work will be accomplished, and accurately reflects the work that has been accomplished and how it was sequenced.
1.4.2 Schedule Monthly Updates
Update the Schedule at monthly intervals or when the schedule has been revised. Keep the updated schedule current, reflecting actual activity progress and plan for completing the remaining work.
1.4.3 2-Week Look Ahead Schedule Prepare and issue a 2-Week Look Ahead schedule prior to the weekly coordination meetings to provide a more detailed day-to-day plan of upcoming work identified on the Construction Schedule.
Additionally, include upcoming outages, closures, preparatory meetings, and initial meetings. Identify critical path activities on the 2-Week Look Ahead Schedule.
1.5 SUPERVISION
1.5.1 Superintendent Qualifications
Provide a project superintendent with a minimum of 5 years’ experience in construction with at least 3 of those years as a superintendent on projects similar in size and complexity. The individual must be familiar with the requirements of EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. The individual must be capable of interpreting a critical path schedule and construction drawings. The qualification requirements for the alternate superintendent are the same as for the project superintendent. The COR may request proof of the superintendent's qualifications at any point in the project if the performance of the superintendent is in question. For projects where the superintendent is permitted to also serve as the Quality Control (QC) Manager as established in Section 01 45 00 QUALITY CONTROL, the superintendent must have qualifications in accordance with that section.
1.5.2 Minimum Communication Requirements
Have at least one qualified superintendent, or competent alternate, capable of reading, writing, and conversing fluently in the English language, on the job-site at all times during the performance of Contract work. In addition, if a Quality Control (QC) representative is required on the Contract, then that individual must also have fluent English communication skills.
1.5.3 Duties
The project superintendent is primarily responsible for managing subcontractors and coordinating day-to-day production and schedule adherence on the project. The superintendent is required to attend partnering meetings, and quality control meetings. The superintendent or qualified alternative must be always on-site during the performance of this contract until the work is completed and accepted.
W912DW25B0005 01 11 00 - 3
1.5.4 Non-Compliance Actions
The Project Superintendent is subject to removal by the Contracting Officer for non-compliance with requirements specified in the contract and for failure to manage the project to ensure timely completion. Furthermore, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to such stop orders is acceptable as the subject of claim for extension of time for excess costs or damages by the Contractor.
1.6 PROJECT SPECIFIC PHYSICAL DATA
Data and information furnished or referred to below is for information only. The Government will not be responsible for any interpretation or conclusion drawn by the Contractor from this data and information. See FAR Clause 52.236-4 PHYSICAL DATA for additional information.

a. 2396 G 2005, PID BBBL38. The benchmark is a 9/16" stainless steel deep rod driven 16.63 m (54.6 ft) to refusal, located just south of the Quileute Marina Boat Ramp, 3.7 m (12.1 ft) north of a utility pole, 2.8 m (9.2 ft) west of the centerline of the asphalt walk 0.62 m (2.0 ft) east of the orange witness post, and 0.15 m (0.5 ft) below the natural grade.
b. 944 2396 TIDAL 7, PID SD0158. To reach the station from the junction of SR101 and SR110 (La Push Rd.) north of Forks; go southwesterly 13.8 miles where the road turns right and becomes Alder Street; then left on river street to a three-way intersection at Main Street; then turn left on Main Street; go approximately 200 feet to an asphalt drive leading to a two-story wooden

STRUCTURE EVALUATION SUMMARY
This concrete box girder bridge is in fair condition overall due to widespread cracking of the superstructure concrete. To help reduce the rate of deterioration, the deck surface should be cleaned and sealed with a silane or siloxane sealant. The condition of the superstructure will continue to be monitored during future inspection cycles.
Other problems include a missing nut from a connection at the west end of the north railing; pack rust with section loss at the rail expansion joints; and accumulated debris and untrimmed vegetation around the bridge.
Corrective measures should be taken as outlined in this report to prevent the possible development of more serious or costly problems in the future.
