About this file

This document is a Contractor Affidavit form required by the Board of Public Education for the City of Savannah and the County of Chatham (Savannah-Chatham County Public School System) in connection with RFQ C27-08 for "As Needed" Geotechnical Engineering, Special Inspections and Materials Testing Services. The affidavit serves as compliance documentation mandated by O.C.G.A. 13-10-91(b) and requires contractors to verify their registration with and authorization to use the federal E-Verify work authorization program. Contractors must provide their federal work authorization user identification number and date of authorization on the form. The affidavit obligates the contractor to continue E-Verify usage throughout the contract period if awarded and requires that any subcontractors engaged for physical performance of services also provide compliant affidavits demonstrating their own E-Verify registration and authorization.

The form requires the contractor's legal name, authorized representative signature, printed name, and title/position, along with execution details including the date and location of signing. Notarization is mandatory, with the notary public required to attest to the sworn declaration under penalty of perjury. This affidavit is a mandatory compliance document for all bidders participating in the prequalification process for this annual "as-needed" contract, which allows SCCPSS to maintain a list of approved vendors for geotechnical engineering and materials testing services for a minimum of one year following vendor prequalification.
