STATEMENT OF WORK

Relocation of Government Property

366TH OPERATIONS GROUP, BLDG 210

Mountain Home AFB ID 83648

11 July 2025

1.0 SCOPE OF WORK: The contractor shall furnish all personnel, equipment, supplies, services and materials for the packing, and moving of office furniture, equipment, and supplies from building 210 and deliver to building 512, and 2215 located on Mountain Home AFB, ID.

2.0 GENERAL DESCRIPTION:

2.1. Services shall consist of a full range of packing, moving, and set-up services to remove existing office furniture, equipment and related supplies from building 210 (See Attachments #1 and #2) to the locations outlined below in paragraph 3, and place as directed by the Contracting Officer’s Representative (COR) for that specific building.

2.2. It is the contractor’s responsibility to coordinate the container size and configuration for each mode of shipment prior to packing containers.

2.3. A COR shall be present at origin and at each destination during the performance of the SOW. Any changes requested by the COR shall be approved by the Contracting Officer (CO) in writing prior to acceptance by the contractor.

3.0 LOCATION AND DESCRIPTION

3.1. All items are currently located at 366TH Operations Group, 725 Alpine Street, Bldg. 210 Mountain Home AFB, ID 83648. The items shall be moved to Building 512, 2nd floor, 366 Gunfighter Avenue, Mountain Home AFB, ID and Bldg. 2215, 1050 Desert Street, Mountain Home AFB, ID. The distance between building 210 and the relocation sites is approximately two (2) miles on a paved road.

3.2. Building 210 does not contain a loading dock or other equipment for loading moving trucks. This building is a single level.

3.3. Parking/loading areas at all locations can support a semi-truck.

3.4. COR will notify the contractor a minimum of five (5) business days in advance to schedule move for each phase listed below to have the items relocated to their new location. All moves will occur between 10 August to 1 September 2025.

3.5. The moves shall be completed in two (2) phases as outlined below:

3.6. Phase One: Select items at Bldg. 210 (Attachment #1), identified by the COR, shall be packed and moved to Bldg. 512, 2nd Floor, 366th Fighter Wing, 366 Gunfighter Avenue on the same day. An elevator is available at this location. 366TH Operations Squadron, 1050 Desert Street (single story) on the same day. This location does not contain a loading dock.

3.7. Phase Two: Select items at Bldg. 210 (Attachment #2), identified by the COR, shall be packed and moved to Bldg. 2215, 366TH Operations Squadron, 1050 Desert Street (single story) on the same day. This location does not contain a loading dock.

3.8. Government personnel shall be responsible for having all drawers, cabinets, bookcases empty and contents packed in boxes ready for transport.

3.9. Government personnel shall be responsible for transporting their own laptops, and monitors off their desks to the new location.

4.0 ESTIMATED WEIGHT: 15,000 lbs. Inventory lists at Paragraph 9.

4.1. Approximate weight to pack and move: 13,000lbs

5.0 TAGGING, INVENTORYING, AND PACKING LISTS

5.1. The contractor shall prepare an accurate, legible and descriptive inventory.

The contractor shall count and examine all goods tendered for packing and make written exception for any goods not in apparent good order. All tagging shall be accomplished prior to packing or moving furniture or equipment from Building 210, with the approval of the COR and other designated government representatives.

5.2. Removal of Debris. The contractor is required to remove any excess containers, packing materials, and other debris accumulated during the performance of this SOW from all facilities and surrounding area.

5.3. Contractor shall furnish padding and/or other protective material for furniture and equipment during move.

5.4. Contractor shall furnish plywood, vinyl, heavy gauge plastic or Masonite runners as required to provide protection to the floors and carpet.

5.5. Disassemble and/or reassemble items when necessary for safe and efficient transportation to the new location, as identified above.

5.6. The contractor shall provide closed-type vehicle(s) of sufficient size to accommodate the shipment and properly licensed personnel to operate such vehicle(s).

The vehicle(s) shall be in good repair and in compliance with all federal, state, and local laws, and regulations for operation.

5.7. All vehicles used in relocations shall be equipped with clean pads, covers, and other protective equipment to ensure safe transit and delivery of property. Protective covering shall be used during the loading and unloading operations in the event of inclement weather.

6.0 DAMAGES

6.1. The contractor shall be responsible for damages to flooring, furniture, and equipment resulting from the physical transfer of the property between the three (3) locations.

6.2. The contractor and COR will visually inspect for any damage to items packed and moved. The COR will forward this listing to the CO who will coordinate with the contractor for the repair and/or replacement of the damaged item(s). Except when loss and/or damage arises out of the causes beyond the control and without the fault or negligence of the contractor, the contractor shall assume full liability for all goods and/or damage in the relocation covered by the contract.

6.3. The contractor shall report all incidents of damage or destruction to property, personal injury or death to any person, arising out of and during the performance of this contract. For the purpose of this statement, the term “property” includes equipment, freight, residence or property of any type whether Government owned or not and the term “Contractor” includes the contractor, its employees, agents, or subcontractors.

7.0 GENERAL INFORMATION:

7.1. DUTY HOURS: Installation normal operating hours are 7:30 a.m. through 4:30 p.m.

Monday through Friday, except Federal holidays or when the Government facility is closed due to 366 FW Family or Down Days or during adverse weather delays/closures.

Local or national emergencies may also affect access to base facilities. Any work requested or performed outside of normal business hours shall be coordinated and approved with the CO PRIOR to the work being performed.

7.2. Recognized Federal Holidays

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the contractor will be responsible for rescheduling services for the first day after the holiday observance.

7.3. A Pre-Performance Conference will be held approximately one (1) week after contract award (ARO).

7.4. Contract Manager and Alternate. Contractor shall appoint in writing to the CO within 10 business days following contract award a contract manager and alternate(s). Any changes to this letter of appointment shall be provided to the CO within three (3) business days. These individual(s) will be responsible for contract performance IAW the PWS. The contract manager or alternate will have full authority to act for the contractor on all contract matters. The name of this person, and an alternate or alternates, will act for the contractor when the primary is absent.

7.5. Contractor Employees. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor, by the Contracting Officer, as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. The contractor shall not employ any person who is an employee of the United States government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7 and the Joint Ethics Regulation. If prior military, the employee must have been discharged under honorable conditions with no administrative discharge actions taken against them.

7.6. All contractor personnel must present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing contractor-furnished clothing bearing the company name and/or logo. Dress of office workers will be in good taste and conducive to working with the general public. The contractor will identify himself as a contractor in meetings, telephone conversations, and e-mails and in any correspondence with the Government.

7.7. All employees shall fluently read, write, speak, and understand the English language.

7.8. Removal of Contractor Employees. The contractor shall utilize only experienced, responsible, and capable employees in the performance of this contract.

The Contracting Officer may require the contractor to remove his/her employees who endanger persons or property, or whose continued employment under this contract is inconsistent with the interests of military security.

8.0 SECURITY AND SAFETY REQUIREMENTS:

8.1. General. Contractor personnel employed under this contract shall meet the requirements of all security requirements addressed in this PWS. The 366th FW Information Protection (IP) office will provide oversight as the Servicing Security Activity (SSA). Sponsoring Air Force organization will ensure Integrated Visitor Groups complete all necessary security training, i.e. Information, Physical Security and Anti- Terrorism. The contractor shall ensure requirements for obtaining Contractor personnel security clearances/background checks, protecting Government property, safeguarding protected information/materials, and shielding Government AISs and non-automated information systems are met. The Contractor shall also perform in accordance with the Compliance with National Industrial Security Program Operating Manual (NISPOM).

NOTE: Not all security requirements will apply to every contract. Please reference contract agreement to determine appropriate requirements.

8.2. Notification of Government Security Activity. Upon performance on the military installation, the contractor will notify the SSA and/or host unit security assistant (USA) 30 days prior to contractor operations. In the event the contract starts within 30 days, the contractor will need to notify the SSA within 72 hours of contract start. The contractor shall furnish the SSA/host USA at the performance location all required information (identifying information for personnel working the contract, contract information, etc.).

8.3. Security Agreement. If deemed as a long-term contract the contractor and sub-contractors shall enter into a security agreement with host base SSA, by reviewing and signing the local security procedures documentation. This agreement shall outline how the contractor integrates security requirements for contract operations with the AF to ensure effective and economical operation on the installation.

8.4. Listing of Employees. The contractor shall maintain a listing of all employees performing work under this contract. The list shall be validated and signed by the Contracting Officer Representative (COR), and provided to the Contracting Officer (CO), SSA and host unit supervision. The contractor shall update the list when employees move positions, are replaced, or upon request. The Contract Manager (CM) will also provide visit notification identifying all integrated personnel regardless of clearance level. The list shall include the employees’ Names, Location of Performance, and Security Clearance.

8.5. Contractor Removal. The CO may direct the removal of a contractor employee from this contract for reasons of security or misconduct. All contractor personnel are subject to Security Forces jurisdiction and the authority of the installation commander while on USAF installations.

8.6. Contractor Integration. Contractor personnel will be integrated into individual host unit organizations as applicable to their respective functional position. The contractor will adhere to all wing and squadron level security policies and directives.

Respective unit security assistants shall provide security administration, to include training and badging.

8.7. Security Support Provided by AF. Security support is provided by the AF to the contractor, including use of base destruction facilities, reproduction facilities, security badges, base visitor control, investigation of security incidents, base traffic regulations, the use of security forms and conducting inspections.

8.8. Off-Installation Security. In the event temporary work is required at locations other than Mountain Home Air Force Base (MHAFB), the contractor will ensure applicable security measures and processes are established with the governing SSA or government agency. Performance intended to exceed 90 days will require revision of the contract.

8.9. Pass and Identification Items. The Prime Contractor shall ensure the pass and identification items required for contract performance are obtained for employees. Base identification badges will be furnished by the SSA and/or host USA for each contractor employee upon presentation of evidence of assignment. Other required badges will be furnished by the Government as necessary to complete task requirements.

8.10. Base Access. The Contractor shall provide a completed 366 SFS Form 30 for each employee requiring installation access to the Contracting Representative/Sponsor within five (5) business days following contract award. The Contracting Representative/Sponsor will complete the SF From 30 and forward to the Visitor Control Center (VCC), IAW current base access requirements, 14 days before base access is requested. Failure to meet the14 day deadline may result in delayed base access. NOTE: The SF From 30 must include the contract number.

8.10.1. The VCC will complete the required base access background checks as indicated on the SF From 30 and will notify the Contracting Representative/Sponsor of the status of the access requests. After the base access request is approved, the Contracting Representative/Sponsor shall notify the Contractor, who will notify their employees that they can obtain their installation access ID at the MHAFB VCC at Bldg.

8.10.2. The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Action (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding For Official Use Only (FOUO) material.

8.10.3. Contractor personnel shall report to an appropriate authority any information or circumstances which they are aware of and may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

8.10.4. The Contractor shall be responsible for safeguarding all Government property. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

8.11. Retrieving Identification Items. The Contractor shall retrieve all identification items from employees who depart for any reason before the contract expires (e.g., terminated for cause, retirement, etc.) Additionally, the Contractor shall retrieve all identification items from employees ,or have the individuals surrender the identification media to the VCC, at termination of the contract. Personnel who still base access for other contracts must reapply for a new base access pass based on that contract.

8.12. Mutable Identification Items. IAW DoDM-5200.08V3_AFMAN31-101V3 and DoDM 1000.13V1, individuals holding more than one acceptable credential must use the credential most accurately depicting the capacity in which the individual is acting for the specific visit to the installation. The Contracting Representative/Sponsor must ensure all personal are briefed on this requirement.

8.12.1. Personnel (e.g., contractors) in possession of another acceptable credential (e.g., retired identification, family member identification, etc.) must complete an SF From 30 for issuance of a new contractor identification for specific period of the applicable contract.

8.12.2. Personnel not complying with this standard can be subject to administrative actions and could be barred from the installation.

8.13. Security Clearance/Background Check. When applicable all personnel performing on this PWS will have a security clearance/background check equivalent to the requirements listed in the contract. Personnel will complete the appropriate documentation (SF-85, Questionnaire for Non-Sensitive Positions or SF-86, Questionnaire for National Security Positions) through National Background Investigation Services (NBIS) website. Documentation of a completed/adjudicated application is required prior to issuance of a Common Access Card (CAC).

8.14. Controlled/Restricted Areas. The Contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. The Contractor must request specific access to Restricted/Controlled Areas through the appropriate government representative at least 2 weeks prior to requiring access. The government representative will ensure access to Restricted/Controlled Areas is approved by owning agency and that any required training and Entry Authority Lists (EALs) are completed prior to contractor personnel entering the Restricted/Controlled Area. The Contractor is responsible for ensuring all personnel accessing the Restricted/flightline Controlled Area meet all access requirements. The contractor will be provided a format for the EAL as needed. When applicable an AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued.

8.15. Controlled Unclassified Information. The contractor shall comply with DoDI 5200.48_DAFI16-1403, Controlled Unclassified Information, for identification, protection, and training requirements of CUI. The contractor shall be responsible for training their personnel and accomplishment of the out-processing procedures identified in DoDI 5200.48_DAFI16-1403. The contractor shall comply with DoDM 5400.07_AFMAN33-302, Freedom of Information Act (FOIA) Program, requirements.

Unclassified DoD information not approved for public release on non-DoD Information Systems will be protected IAW DoDI 8582.01, Security of Non-DoD Information Systems Processing Unclassified Nonpublic DoD Information.

8.16. Foreign Disclosure. Due to the nature of the MHAFB mission, foreign integration and sensitivity of information and materials, the contractor will ensure all personnel are aware of and comply with foreign disclosure procedures. Release of CUI and Classified Military Information must be approved by the 366 FW Foreign Disclosure Officer

(FDO).

8.17. Operations Security (OPSEC). The contractor shall comply with 366 FW OPSEC requirements and countermeasures to protect critical information as defined in AFI 10-701, Operations Security. Contractors shall receive unit specific OPSEC briefing prior to the start of work. Briefing documentation must be completed by the respective unit OPSEC coordinator. The unit OPSEC personnel will provide guidance on protecting critical information, training materials and other instructions as needed. The contractor will comply with the 366 FW OPSEC Policy that imposes a 100% shred requirement of all government related paper products using NSA approved devices only.

8.18. Antiterrorism (AT). IAW DFARS 252.204-7004, Antiterrorism Awareness Training for Contractors, and DoDI O-2000.16 Volume 1, DoD Antiterrorism Program Implementation: DoD Antiterrorism Standards, all contractor personnel who require routine physical access to MHAFB shall complete Level I AT Awareness Training within 30 days of requiring access and annually thereafter. Routine physical access is defined as any contractor issued a Common Access Card (CAC), any contractor having installation access for a year or more, any contractor requiring a security clearance, or any contractor requiring access to specific secure areas (e.g., SAPF, SCIF, etc.).

8.18.1. Level I AT training shall be complete through a DoD-sponsored/certified computer or web-based distance learning instruction, or under the instruction of a Level I AT Instructor. Contractor personnel can complete DoD Level I AT Training at https://jko.jten.mil/. If the Contractor has personnel who can’t access the DoD Level I AT Training site, they notify the COR to arrange access to the training site or to coordinate other Level I AT Training.

8.18.2. The prime contractor must maintain copies for all Certificates of Completion for AT Level I, and the appropriate Contracting Officer Representative will conduct checks, prior to initial installation access and annually thereafter, to ensure all personnel have completed the training.

8.18.3. All personnel, to include contractors, on MHAFB will comply with directed/implemented Force Protection Condition (FPCON) and/or AT measures, unless specifically exempted by the 366 FW Commander (CC). Contractor personnel will submit requests for exceptions to FP/AT Measures through the COR, who will submit the request to 366 FW Antiterrorism Officer (ATO) for submittal to 366 FW/CC.

8.18.4. During periods of increased threat or raised FPCONs, contractor access to the installation, or specific parts of the installation, may be limited or denied. The COR will, when possible, notify contractor personnel before the limitations/restrictions are implemented. Contractor personnel will coordinate with the COR regarding possible affects limited/restricted access will have on contract completion. Contractor personnel will coordinate with the COR on assignment of Mission Critical Contractor status as needed.

8.18.5. During periods of increased threat or raised FPCONs, personnel, to include contractors, may be required to be escorted while on MHAFB. When being escorted, personnel will always remain with their escort. If personnel become separated from their escort, they will remain where they are and notify the 366 Security Forces Squadron (SFS) at 208-828- 2256/2257/2258.

8.18.6. The Contractor shall include the DFARS 252.204-7004, Antiterrorism Awareness Training for Contractors, clause as well as all other above identified AT items, in all subcontracts, including subcontracts for commercial items, when subcontractor performance requires routine physical access to MHAFB.

8.19. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

8.20. Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

8.21. Key Control/Access Badge Control. The Contractor shall establish and implement methods of making sure all keys/badges issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The Contractor shall not duplicate any keys issued by the Government.

8.22. Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.

8.23. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on DOD Installations.

8.24. Domestic Violence/Lautenberg Amendment. Contractor personnel who have duties in, have access to, or are otherwise involved in the storage, issue, disposition of, or transporting firearms or ammunition shall complete DD Form 2760, Qualification to Possess Firearms or Ammunition, annually. The DD Forms 2760 shall be maintained by the CM and be made available to the KO and COR office upon request.

Any person who has been convicted of a crime of domestic violence is not eligible to perform these duties. All personnel have an affirmative, continuing obligation to inform their supervisors if they have, or later obtain, a qualifying conviction.

Candidates selected to fill these positions certify they have not been convicted of a crime of domestic violence, or who is subject to a protective order for domestic violence prior to assuming the duties of the position. These activities are covered by 18 U.S.C. § 922.

9.0 ATTACHMENTS
