About this file

This is a Short Term Contractors Template CBP Contractor Information Sheet, which serves as a visitor access form for contractors and personnel requiring temporary access to U.S. Customs and Border Protection facilities. The form collects comprehensive information about visiting contractors, including company details, sponsoring CBP employee information, visitor biographical data (name, date of birth, citizenship status, place of birth, occupation), and identification documentation (visa, passport, driver's license, and social security number). The template accommodates up to 80 individual visitors per submission.

The form establishes mandatory security procedures requiring submission 72 hours prior to visitor arrival for approval through the Office of Professional Responsibility Security Management Division via email (SMDSECURITYSERVICESBRANCH@cbp.dhs.gov) or fax (202-344-1860). All visitors must present U.S. Government-issued photo identification and check in/out at the security desk. Visitor escorts are required at a ratio of one escort per four visitors (1:4), though visitors with unvetted status may require one-to-one escort coverage. Escorts must maintain constant supervision of visitors at all times, and any unescorted visitors found in CBP space will be reported to security and removed. The submission is governed by the Privacy Act of 1974 and is used to conduct preliminary security vetting for authorized facility access.
