About this file

This is a Short Term Contractors Template Visitor Escort Information Sheet used by U.S. Customs and Border Protection (CBP) for managing contractor and visitor access to CBP facilities. The form captures comprehensive contractor company information, CBP location details, and designated escort personnel responsible for supervising visitors on-site. It requires detailed personal information for all visitors, including full names, dates of birth, citizenship status, place of birth, occupation, and identification document numbers (passport, visa, driver's license, and social security number).

The document establishes strict security protocols requiring mandatory submission of visitor information 72 hours prior to arrival, with submissions directed to the Security Management Division via email or fax. All visitors must present U.S. Government-issued photo identification and check in/out at the security desk. Escorts are assigned at a ratio of one escort per four visitors (1:4), though higher-risk vetting status may require one-to-one escort ratios. Escorts must maintain constant supervision of visitors at all times, including during departure. Any unescorted visitors found in CBP spaces will be immediately reported to security and removed. The form processes visitor information under the Privacy Act of 1974 to conduct minimum security checks before granting limited facility access, with failure to provide required information resulting in access denial.
