About this file

This is a Past Performance Questionnaire template designed for the RFP 70B03C26R00000045 issued by U.S. Customs and Border Protection (CBP) for Non-Intrusive Inspection (NII) Relocatable Passenger Vehicle Scanning Systems (RPVSS). The questionnaire is completed by government agencies or commercial organizations with knowledge of a contractor's past performance and must be submitted by April 1, 2026 at 5:00 PM Eastern Daylight Time to benjamin.j.dorgan@cbp.dhs.gov. The form requests identifying information including the contractor name, contract number, period of performance, dollar value, and description of products or services provided.

The questionnaire evaluates contractor performance across four key areas using a three-tier rating system (Superior, Satisfactory, Unsatisfactory): ability to meet requirements, timeliness of product delivery, quality of shipments including packing and marking, and overall performance rating across requirements, schedule, and price. Performance definitions are provided to guide assessments, with Superior indicating performance exceeding contractual requirements with few minor issues, Satisfactory indicating meeting contractual requirements with minor issues resolved effectively, and Unsatisfactory indicating failure to meet contractual requirements with serious unresolved problems. Respondents are encouraged to provide detailed narrative explanations for their ratings on additional pages as needed.
