About this file

The document is a Past Performance Questionnaire for a Navy contract solicitation (N6883625QS012) issued by NAVSUP FLC Jacksonville. The questionnaire is designed to evaluate a contractor's performance across 26 different criteria, ranging from management capabilities and quality control to project supervision and delivery schedule adherence. Evaluation levels include Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), and Unsatisfactory (US), with space for detailed comments on any exceptional, marginal, or unsatisfactory ratings. The form requires the evaluator to provide comprehensive feedback on the contractor's technical abilities, problem-solving skills, cooperation, reporting accuracy, and overall performance, with a final section for a narrative summary and potential recommendation for future contract awards.
