About this file

This document is a Past Performance Questionnaire (PPQ) for a federal contract (N6883625QS013) related to a Microfocus Fortify software subscription renewal. The questionnaire is designed to evaluate a contractor's performance across 26 specific criteria, including management capabilities, quality control, problem-solving, delivery schedules, reporting, and overall contract execution. Performance is rated using a six-level scale from Exceptional (E) to Unsatisfactory (US), with the evaluator expected to provide detailed explanations for any ratings at the exceptional, marginal, or unsatisfactory levels. The questionnaire is to be completed and submitted by 5/13/2025 to Navy contract officials Yaynel Yambo and Robert Edwards, with the responses serving as a critical input for future contract considerations and vendor performance assessments.
