About this file
This is a Respondent Questionnaire attachment for Request for Proposal BOE #2026-02 issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the Board of Ethics. The RFP seeks qualified firms to provide technical and professional services, specifically a preliminary written legal analysis of an investigation into alleged violations of the City of Detroit ethics ordinance. The questionnaire requires respondents to provide comprehensive organizational information, including the single point of contact for the RFP process and project manager or partner details. Respondents must document their background information, including legal business name, principal place of business, business entity designation, Federal Employer Identification Number, years in business, and current employee counts, with specific attention to Detroit resident employment and participation. The questionnaire mandates submission of at least three professional references from the last five years that attest to relevant experience managing contracts of similar size and scope.
The questionnaire requires respondents to provide detailed organizational documentation, including an organizational chart identifying key personnel, individual resumes for all key personnel, evidence of State Bar of Michigan membership for legal professionals, and documentation of employee availability and experience. Respondents must disclose any proposed subcontractors with corresponding reference forms and verify that subcontractors are not on the City of Detroit's debarred and suspended suppliers list. The document also requires disclosure of any claims, lawsuits, contract terminations, and debarment or suspension history for the respondent or associated parties. Additionally, respondents must disclose whether their proposed solution involves artificial intelligence, which would require completion of the City's AI Factsheet as a condition of award. The questionnaire indicates willingness to enter Priority Hire Agreements with Detroit at Work to prioritize hiring Detroit residents and requests information on specific credentials or training to be provided to new hires.
