About this file
This document is a set of standard solicitation response forms required by the Tohopekaliga Water Authority (Toho) for vendors responding to a formal solicitation for professional services related to the Southside Water Reclamation Facility Expansion project in Florida. The project encompasses survey, geotechnical investigation, preliminary design, final design, permitting, and limited construction administration necessary to expand the existing Southside Water Reclamation Facility from 7.6 to 9.0 million gallons per day. Vendors must submit all applicable response forms as part of their solicitation packages, with specific acknowledgments required regarding addenda issued during the solicitation process.
The solicitation response forms establish compliance requirements across multiple regulatory areas, including non-collusion and employee interest certifications, E-Verify registration and employment eligibility verification, vendor eligibility certifications under Florida Statutes Sections 287.133 through 287.138 regarding public entity crimes, discriminatory practices, antitrust violations, scrutinized companies and boycotts, and foreign countries of concern. Vendors must also acknowledge receipt and understanding of confidential structural records protections under Section 119.071(3)(b), Florida Statutes, and provide common carrier certifications. All forms require notarization and certification under penalties of perjury by an authorized representative of the vendor. Vendors must demonstrate they are not on any convicted vendor list, discriminatory vendor list, or antitrust violator list, and must comply with Florida's employment eligibility verification requirements, including E-Verify registration for themselves and any subcontractors.
