About this file
This is an Internal Inspection Report for a 30/40/50 MVA transformer located at Oliver Substation in Lubbock, Texas, prepared for Lubbock Power & Light (LP&L) in response to RFP# 7123-26-ELD concerning transformer repair services. The inspection was conducted on January 7, 2026, following a feeder fault, detection of high acetylene levels in the tank, and test data irregularities at the Oliver Substation facility located at 10602 Indiana Ave, Lubbock, TX 79423. The transformer, manufactured by Virginia Transformer with serial number 48030MA047U-HA887A, is a 115/69kV to 12.47kV unit with an oil volume of 7,785 gallons. The inspection revealed significant internal damage requiring comprehensive repair efforts, with the full extent of damage unable to be determined without winding disassembly due to the location and nature of the fault.
The inspection identified multiple critical deficiencies requiring remediation: the A phase lead bundle was not properly secured by the wooden clamp block with missing fiber reinforced polymer rod and hardware, copper debris was discovered surrounding the A phase leads, burnt insulating tape debris was scattered throughout the transformer interior, and carbon residue was visible surrounding the leads indicating internal winding fault evidence. Additional findings include loose and potentially missing hardware on the CT passthrough block clamping support studs, loose hardware on CT clamping support studs, and debris accumulated at the tank bottom. The B and C phase windings showed no visible damage beyond previously noted areas. LP&L performed initial oil sampling and load tap changer inspection prior to the internal assessment, determining that the main tank required draining and internal inspection to assess the full scope of necessary repairs.

INTERNAL INSPECTION
Job # 12505518
Equipment Info: T4 OEM – Virginia Transformer
SN – 48030MA047U-HA887A
Voltage Class – 115/69kV – 12.47kV
MVA – 30/40/50
Oil Volume – 7,785 Gallons
Oliver Substation
10602 Indiana Ave
Lubbock, TX 79423
Reason for inspection:
Feeder fault
High level of Acetylene in the tank
Test data irregularities
1/7/2026
Inspection Performed 1/7/2026
Lubbock P&L Oil Sample Data:
The oil sample and pretesting were performed by LP&L before RMS arrived. The LTC was drained and inspected by LP&L. LP&L determined that the LTC was unharmed and that the transformer main tank would have to be drained and inspected.
LP&L Pre-Test Data:
Once the manhole was removed the first thing the team noticed was that the A phase lead bundle was not secured by the wooden clamp block and fiber reinforced polymer rod and hardware was missing.
Copper debris was found immediately surrounding where the A Phase leads travel from the LTC to the inside of the winding. Also burnt insulating tape debris was found scattered around the inside of the transformer.
The source of the debris was further investigated. Damage was visible from where the LV leads exit the winding below the frame assembly. Inside the space between the wooden clamping block and the vertical winding insulation barrier there is enough room to shine a light and take pictures of what looks to be evidence of the fault inside the winding (See red arrow below). Carbon can be seen surrounding the leads in the picture.
The rest of the windings, core, frame, preventative auto, DETCs, CTs, and series reactor were inspected:
i
The internal side of the CT passthrough block has loose hardware and some potentially missing. It was either previously never installed or not torqued properly:
The CTs had some loose hardware on the clamping support studs:
The transformer has debris scattered around the bottom of the tank:
The B and C windings were inspected and there was no visible evidence of damage from anywhere but previously mentioned. Nowhere else on A phase did the team find evidence of further damage. This does not rule out the possibility of further damage hidden by insulation or in otherwise inaccessible locations. Due to the location of the failure; the full extent of the damage is unable to be determined without winding disassembly
Thank you for your attention to this report. We look forward to continuing our work together and supporting any decisions that follow.
Thanks, Jarret Schwartz
