Title: LES Supplemental Health Insurance
Questions and Answers, Volume 3, dated May 21, 2020
Proposals receiving end date May 26, 2020/17:00h
Q1: In L.4.3 part 2, point 4 there is a request for “Describe the pool(s) of coverage the offeror uses to administer its insurance; which pool of coverage the employees under this requirement will be contained, and what percentage of that pool they would represent. “ Our questions is if this refers to our employees who work on this insurance at our firm on regular basis, and to establish a % from them, who will be dedicated to administer this contract?
A1: Yes. Please describe your company’s employee pool that administer insurance. In case you are awarded a contract please state a percentage of your employees that will administer the contract.
Full text of the clause from the solicitation follows:
L.4.3. Part 2, Para 4. Describe the pool(s) of coverage the offeror uses to administer its insurance; which pool of coverage the employees under this requirement will be contained, and what percentage of that pool they would represent.
Q2: L.4.3.4 refers to employee pool. This means that the number of your employees who will be eligible for insurance at the beginning will represent for example a group of 250 people, and we have to submit boundaries (10±) in which the price will remain the same. For example if a total of 260 employees is through-out the year the price remains the same, and if you go above 260, you will pay the X price per employee?! This is also valid if you decrease the number of employees, to below 240 for example?
A2: Section Section L.4.3. Volume 3, Technical Proposal refers to your proposal and how will you react in case you are awarded a contract. Section L.4.3.4 refers to your engagement in case the economic price adjustment clause becomes effective (Effective in situations of major local economy drop). Please state in your proposal a percentage of your employees that will administer this contract in situations where economic price adjustment clause becomes effective.
L.4.3.4 Employee Pool
The offeror shall describe the pool that will apply to the employees under this contract. The offeror will describe the size of the pool, whether it is a mixture of commercial and government
(if applicable), alternative pools that are available in the event the economic price adjustment clause becomes effective.
Q3: In what form should we submit our offer because we find in various sections blank tables that we need to fill. Should we print the whole document 59 pages, signed every page, fill the

required tables/fields or we should put this fields/tables in to separate document together with the other documents.
A3: Potential offerors may print the solicitation fill-in the required blocks, make 4 copies (Per block 9 from the solicitation) and submit to the address in block 9 form the solicitation.
Q4: Please explain a little bit more about the retention amounts and price adjustment in sections
B3 and B4.
A4: Retention is an amount specified in the insurance policy that must be paid by the insured before the insurance policy will respond to a loss.
Q5: The question that we have is regarding the Block 15 in SF-33 form, what is supposed to be filled in the fields CODE and FACILITY of this section.
A5: Blocks CODE and FACILITY refer to the U.S. Commercial and Government Entity known as (CAGE) code for services. Please refer to Section K.12 (3) for instructions.
If you know your Commercial and Government Entity (CAGE) code and you have a remittance address different from 15A, you should insert the CAGE code in 15A and place an X in 15C.
