Attachment 2: Request for Sole Source Justification Format >SAT
DEPARTMENT OF VETERANS AFFAIRS
Justification and Approval (J&A) For
Other Than Full and Open Competition (>SAT)
Requisition ID: 402-20-2-100-0038
Contracting Activity: Department of Veterans Affairs, VISN 01, VA Maine Healthcar Systems. 1 VA Center, Augusta, Maine 04330
Nature and/or Description of the Action Being Processed:
Maine VAMC needs an Ultrasound System with Super-Harmonics because it is a way for Anesthesiologists and Nurse Anesthetists to see and guide their usual blocks with more enriched detail. This not only makes it faster, but much safer for the patients. The high resolution allows the doctors to see detail better and will allow many clinicians to perform additional procedures presently off-limits due to the limited technology in our existing machines." The portable solution provides the advanced features and functionality needed to perform fully diagnostic exams, and easy enough for the beginner toscan with factory presets and familiar touchscreen capabilities."
Description of Supplies/Services Required to Meet the Agency's Needs:
The Smart 3200T NexGen Super-Harmonic Ultrasound is a grab and go imaging solution featuring crystal clear images, frame rate stability, and an intuitive interface. The ultrasound tablet 2D Digital imaging includes; color Doppler, pulsed wave Doppler, directional power Doppler, continues wave Doppler, triplex dynamic depth resolution, exclusive enhanced needle visualization, to mention a few.
The best part about the whole system is that it has zoom, split screen, full screen, Post processing with touch screen capabilities.

FAR13.5 Simplified Procedures for Certain Commercial Items:
5. Demonstration that the Contractor's Unique Qualifications or the Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): This project will provide three state of the art Ultrasounds for the VAMC Togus OR. This is the standard of care in the outside facility.
The Terason uSmart 3200 Next Gen ultrasound system has many features that the competitors do not have, making it a far superior choice in the world of ultrasound equipment. Aside from its slim, lightweight design, this touch screen tablet uses "Super-Harmonic lmaging" which provides crystal clear imaging. Terason also uses "Enhanced Needle Visualization" technology. ENV is second-generation technology, providing better needle guidance at steeper angles without image degradation or drop in frame rates. There is no need to pre determine insertion angles, thus reducing procedure time, improving accuracy, and decreasing potential complications. Clear imaging and needle visualization are the two most important pieces of using an ultra-machine.
There are many other features and benefits of using the Terason 3200T NexGen Ultrasound, but these two alone make it a leader amongst the competitors. We currently have one on station and by adding three more it will standardize the currently system already in place.
Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: Only one source is available. Terason, Inc., a division of Tetrarch Corporation, is the ole ource, small business manufacturer for the uSmart 3200T NexGen Super-Harmonic Ultrasound.
Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The cost was compared to GSA Prices offered by a reseller of the uSmart 3200T NexGen Super-Harmonic Ultrasound. The technology and the best value are exclusive to Tetrarch Corporat on (Terason).
s. DDescription of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: The vendor states that it is the sole source for this technology. Traditional market research was conducted. There were no mandatory sources, SD/VOSB (Service-Disabled/Veteran-Owned Small Business) vendors, or distributors who could source this equipment. The manufacturer is a small business manufacturer. This equipment is peculiar to Terason. Terason is unique in its capability to fulfill the requisition.
Any Other Facts Supporting the Use of Other than Full and Open Competition: According to 13 CFR 121.406(b), because Terason is a small business manufacturer of a domestic product, sole sourcing to Terason will satisfy set-aside requirements per F 19.202-2;
no SD/VOSB vendors have the capability to provide this requirement.
Listing of Sources that Expressed, in Writing, an Interest in the Acquisition: None.
Received only three responses from SD/VOSBs. All declined to quote, indicating that they are not capable.
A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required:
